Bayshore's Integrated Care Solutions is seeking a Manager of Clinical Practice to lead our Bundled Care Program, supporting clients and caregivers through high-quality, community-based care. In this leadership role, you'll collaborate with interdisciplinary teams, mentor frontline staff, and ensure care delivery meets the highest standards of excellence, compassion, and reliability.

This position requires full-time, in-person presence five days a week at our Markham office.

What You'll Do

• Provide fill-in support across multiple programs and serve as the go-to resource for backup coverage, ensuring continuity of care and team support during absences or high-demand periods.
• Lead holistic care delivery with nursing, PSWs, allied health professionals, and social workers.

• Partner with hospitals, HCCSS, and community organizations to ensure safe transitions.

• Ensure compliance with federal/provincial standards and Bayshore policies.
• Conduct case reviews and monitor client outcomes.
• Design individualized care plans for safe discharges and continuity of care.
• Address patient/family concerns with empathy and professionalism.
• Support staff development through training-focused home visits.
• Analyze intake processes and improve service access and efficiency.
• Oversee client assessments and ensure accurate documentation.
• Collaborate on program metrics and escalate concerns as needed.
• Participate in case conferences, team meetings, and professional development.
• Be available for flexible scheduling and on-call rotation.

Qualifications : Qualifications - External

What You Bring

• 5 years in healthcare, ideally in home care and hospital settings.
• Registered Practical Nurse with current CPR certification.
• Strong knowledge of community nursing, discharge planning, and Ontario's health system.
• Proven leadership and mentorship skills.
• Familiarity with RAI tools (an asset).
• Valid driver's license and reliable vehicle.
