Community Reintegration Worker

SLCS is currently seeking a full-time Community Reintegration Worker for the HART Hub. The HART Hub (Homeless and Addiction Recovery Treatment Hub) is a low barrier treatment-oriented space offering supports to individuals who are experiencing or at risk of homelessness and who face challenges related to addiction and/or mental health to recover and improve their overall well being and secure long-term housing. As part of this collaborative and client-centered team, you will work alongside external agencies/community partners, healthcare providers, addiction specialists and participants working towards reducing barriers, connecting to resources and supporting access to safe, appropriate and permanent housing.
In alignment with the HART Hub's commitment to trauma-informed, culturally safe, and integrated care, this role will work closely with the HART Hub's lead agency in providing wrap around supports to individuals along their journey, including offering assistance in navigating the Criminal Justice System, connection to community resources in alignment with personal goals, creating individualized support plans and assisting with transition and reintegration into community through stable housing.
This role supports individuals with overlapping and complex service needs, including addiction, mental health, physical health and homelessness. You will have knowledge and expertise of addictions, mental health, the Housing First philosophy and Trauma Informed Care will be critical. The role requires an ability to work within an interdisciplinary team, creating strong rapport and professional relationships with various community partners. You will be required to problem solve and be able to engage in crucial conversations to ensure seamless service coordination. Strong communication and coordination skills will be key in providing support to individuals along their HART Hub journey. Knowledge in the development of HART Hubs through the Ontario Government is an asset.
As a representative of SLCS, you will advocate for participants, promote program visibility, and contribute to a collaborative, inclusive and responsive service environment. The role will contribute to the collective mandate of the HART Hub and will meet all regulatory and organizational standards including strong documentation skills, collection of statistical data, responsiveness to participant needs in meeting them where they are at and the development of participant goals and follow through. This position will interact with individuals from diverse populations and cultures and will approach situations from a Restorative lens.
This is a community-based position requiring travel within the London region, where you will engage directly with participants and partners to deliver high-quality, person-centered care. This position is responsible for the safety and security of participants, co-workers and community during employment.
Required Qualifications:
- University Degree or 2 - 3-year College Diploma from an accredited institution in a related field
- 1-3 years' experience working with a diverse population, including individuals impacted by mental illness, justice system involvement, addictions and homelessness
- Standard First Aid / CPR with AED - Level C
- Valid Driver's License and access to reliable vehicle
- Candidates who are proficient in both official languages of Canada will be considered an asset
