Overview
iScope Concussion & Pain Clinics is Canada's national, neuro-focused rehabilitation network. Our multidisciplinary team — including Neurologists, Psychiatrists, Physiotherapists, Occupational Therapists, Chiropractors, RMTs, Clinical Counsellors and Kinesiologists — provides coordinated, evidence-based care to patients recovering from concussion, chronic pain, and complex injuries.
We are seeking a
dedicated and compassionate Intake Coordinator
to join our team. This role plays a key part in helping individuals who have recently experienced a
motor vehicle accident (MVA)
or other injury navigate their rehabilitation journey with care, clarity, and efficiency.
Key Responsibilities
1. Referral Intake & Assessment
- Receive, review, and prioritize incoming referrals for chronic pain and rehabilitation services.
- Assess completeness of documentation and determine clinical urgency.
- Collect and organize medical history, imaging, and previous treatment information.
2. Care Coordination & Scheduling
- Communicate with referring physicians and specialists to clarify information and ensure smooth case transfer.
- Schedule patients with the appropriate providers based on referral details and availability.
- Confirm appointments and provide logistical details and preparation instructions to patients.
3. Patient Communication & Support
- Serve as the main point of contact throughout the intake process.
- Clearly explain next steps, timelines, and expectations to patients and families.
- Ensure patients feel supported and informed during each stage of their care journey.
4. Documentation & Record Management
- Maintain organized and confidential records of referrals, communications, and appointments.
- Track pending referrals, follow up as required, and ensure timely processing.
- Contribute to ongoing quality improvement by identifying workflow efficiencies.
5. Collaboration & Professional Growth
- Work closely with clinicians, administrative teams, and leadership to coordinate care and resolve barriers.
- Participate in team meetings and training sessions to stay current on best practices in pain management and MVA case coordination.
Qualifications
- Diploma or degree in
Kinesiology, Health Sciences, Rehabilitation, or a related field
(preferred). - Experience in
case management, intake coordination, or healthcare administration
. - Strong knowledge of the
Ontario Motor Vehicle Accident (MVA) / Statutory Accident Benefits (SABs)
system is highly desirable. - Excellent communication, organizational, and multitasking skills.
- Proficiency with EMR systems and a commitment to confidentiality and patient care.
