The Wraparound Process is an innovative approach that mobilizes the community to help children, youth and families with multiple, unmet needs, find solutions that will help improve the quality of their lives.
This proven approach surrounds youth, young parents and families with a support team of family members, social workers, public health nurses, faith groups, friends and community members. The support team is chosen by the individual(s) and includes formal and informal members. The team works together to help the young person/family establish their goals and identify action items
The Wraparound Facilitator will work with children, youth and families to help them recognize their strengths mobilize their individual support team and plan around their individual needs.
KEY RESPONSIBILITIES:
Facilitate the Wraparound Process within the context of an Individual and Family Team.
Guide the community team to develop action items based on identified life domain goals:
Develop, implement, monitor and revise Wraparound Strength Needs and Cultural Discovery Plans, Action Plans and Safety Plans when needed.
Facilitate the process in a timely manner while modeling the facilitation role within the team
Maintain documentation and files as required by Lumenus in accordance with Ministry, licensing, and regulatory bodies
Participate in quality assurance, evaluation and research activities, as required.
Ongoing commitment to learning, understanding and incorporating knowledge of Anti- Oppressive Practice
Participate in training provided by Lumenus, which is specific to job requirements (i.e. CPI, First Aid, CPR, etc.);
Perform other duties as assigned.
QUALIFICATIONS:
Minimum 5 years' experience within the social services field;
Undergraduate degree or diploma from a recognized university or college in a field related (i.e., social services, psychology)
Experience or training in Wraparound's high fidelity model an asset;
Demonstrated ability to apply strengths-based approach to work with children and families, preferably with training as a Wraparound Facilitator;
A comprehensive knowledge of the social service sector within the GTA;
Demonstrated sound judgement related to clients and families in high risk circumstances to ensure appropriate safety plans are arranged;
Strong decision-making skills, good judgement and the ability to navigate relationships with community partners. A capacity to build and nurture working relationships with partner agencies
A self-starter with an ability to work independently, as well as within an established team structure
Well-developed computer skills, including proficiency in Microsoft applications
community centered work
Highly developed communication skills, both written and oral
Demonstrated sound judgment and sensitivity to client differences (e.g. race, religion, culture, gender identity and sexuality);
Demonstrated ability to work co-operatively and negotiate effectively within a multi-disciplinary team;
Ability to balance demands/priorities of workload, manage time effectively and operate within the framework of stated expectations.
ADDITIONAL REQUIREMENTS:
Satisfactory clearance under the Vulnerable Sector Screening process;
Travel required to external agency locations (schools, hospitals, family homes, and residential settings) and family homes.
Ability to work flexible hours in order to meet the demands of direct service requirements.
