Health Guide

( 24 hours/week, Permanent + 16 hours/week contract until March 2026)

Langs has grown to become a dynamic, respected multi-service organization that uses a community development approach to provide comprehensive health, social and recreational services that are responsive to the changing needs of our community.

What We are Looking For:

We are currently recruiting for a Health Guide to join our Clinical team for a permanent, part-time position combined with a contract position equalling full-time hours until March 2026. This role is responsible for supporting patients basic needs and coordinating supports and referring to local organisations in support of meeting the patient's needs in regards to all factors of the social determinants of health.

A successful candidate in this role would have:

· Degree or diploma in social services or a related field

· Minimum three years experience in social services, case management, system navigation and service coordination

· Experience with patient advocacy and supporting individuals with multiple, complex needs

· Strong knowledge of community resources, local health care services, provincial and federal documentation

· Previous experience in non-profit sector an asset

· Demonstrated ability to work independently and in a team environment

· Excellent communication and organizational skills with the ability to demonstrate decision making, problem solving and conflict resolution.

· Proficient in use of technology including Microsoft software, database programs; experience with an Electronic Medical Record an asset.

· Class G License and vehicle available for use. Travel is an expectation as this position will be involved in outreach activities

In this role you will be expected to complete the following tasks:

· Work with primary care providers to coordinate supports for basic needs (food, shelter, clothing).

· Support patients to identify needs and goals and assist in developing their care plan.

· Assist with applications and forms (housing, financial supports, education, transportation, etc.).

· Coordinate and advocate between patients and their service providers, ensuring clear, timely communication.

· Conduct home visits, develop Coordinated Care Plans, and maintain regular follow-up with patients and care partners.

· Provide appointment reminders, arrange transportation, and ensure warm handoffs; attend appointments when needed.

· Ability to travel within Waterloo Region and work across various primary care settings.

· Ability to work flexible hours, including one evening per week.

· Other duties as required.

The successful candidate would be eligible for the following benefits, pro-rated to the permanent hours:

· Starting Salary of $24.97-$28.04 Hourly

· 3 Weeks paid vacation, with an increase to 4 weeks after 1 year of employment

· 15 Sick/Personal Days per year (pro-rated to hours worked)

· 10 Professional Development days per year (pro-rated to hours worked)

· Enrolment in Healthcare of Ontario Pension Plan (HOOPP)

· Comprehensive Extended Health and Dental Care Plan including a $1000 Healthcare Spending Account.
