Position Summary

The FASD/PCAP Mentor supports individuals and families affected by Fetal Alcohol Spectrum Disorder (FASD) and provides targeted prevention and intervention services for women at risk of alcohol or substance-exposed pregnancies.

This position focuses on home visitation, community outreach, and case management, offering trauma-informed, culturally safe, and relational support. The Mentor plays a vital role in helping participants set and achieve goals that promote stability, wellness, and healthy family outcomes.

In Edson, this role also includes community capacity-building through awareness, prevention education, and the coordination of local FASD and PCAP supports in partnership with the Willow Winds Support Network (WWSN).

Key Responsibilities

1. Participant Support and Case Management

  • Provide ongoing outreach, intake, and assessment for individuals and families impacted by or at risk of FASD.
  • Develop and implement individualized support and goal plans that recognize each participant's strengths, barriers, and cultural values.
  • Support women in achieving healthy pregnancies and stable living environments through the Parent-Child Assistance Program (PCAP) model.
  • Offer case management for up to three years, including home visits, mentoring, and coordination of community supports.
  • Assist clients with accessing FASD assessment and diagnostic services, post-diagnosis follow-up, and connection to treatment and community supports.
  • Advocate for participants with service systems such as Alberta Health Services, Children and Family Services, Justice, Housing, and Education.
  • Provide transportation or accompany clients to key appointments when necessary.

2. Targeted and Indicated Prevention

  • Provide prevention-based outreach to women of childbearing age who use substances or have a history of alcohol/drug-exposed pregnancies.
  • Deliver culturally appropriate education on the effects of prenatal alcohol exposure and promote healthy pregnancy choices.
  • Support referrals to sexual health, addictions, prenatal care, and mental health services.
  • Assist participants in building stability through recovery, contraception, parenting support, and family planning.

3. Community Capacity Building

  • Build partnerships with local agencies, Indigenous organizations, health and social service providers, and community members.
  • Lead or participate in FASD support groups and attend interagency and network meetings.
  • Plan and promote FASD Awareness Day events and prevention activities in Edson and surrounding communities.
  • Provide education sessions for professionals, schools, and community groups to raise awareness about FASD.
  • Represent HELP and WWSN at public events, ensuring all activities are reported for provincial statistical collection.

4. Program Evaluation and Reporting

  • Maintain accurate participant records, case notes, and data entry in the Online Reporting System (ORS) and internal databases.
  • Collect outcome data and participant feedback to measure program impact.
  • Prepare bi-annual reports, case summaries, and statistical submissions to the Willow Winds Support Network.
  • Ensure confidentiality and compliance with agency and government standards.

Qualifications

  • Diploma or degree in Human Services, Social Work, Psychology, or a related field; equivalent experience considered.
  • 2–4 years of experience providing case management, home visitation, or family support in a community-based setting.
  • Strong understanding of FASD, addiction recovery, harm reduction, and trauma-informed practice.
  • Experience working with Indigenous and rural communities using culturally safe approaches.
  • Ability to work independently and as part of a multi-disciplinary team.
  • Excellent communication, organization, and record-keeping skills.
  • Proficiency in Microsoft Office, Google Workspace, and secure data reporting tools.
  • Proficiency in Apple products including iPhone and MacBook Air
  • Valid Class 5 driver's licence, reliable vehicle, and willingness to travel for outreach.
  • Satisfactory Criminal Record and Vulnerable Sector Checks.

Work Conditions

  • Travel throughout Edson and Yellowhead County; mileage reimbursed per policy.
  • Travel with participants to assessments, medical appointments, treatment, summer camps throughout Alberta.

Performance Indicators

  • Participants demonstrate improved stability, goal progression, and engagement with community supports.
  • Successful completion of individualized PCAP and FASD service plans.
  • Increased awareness and prevention activity in the Edson region.
  • Effective collaboration with WWSN and community partners.
  • Timely and accurate reporting in ORS and agency systems.
