The Community and Population Health Services portfolio supports the advancement of integrated health and social services in hospital and into the community. The Program is responsible for the development and advancement of innovative partnerships with peer, community, academic partners and provides expert leadership in the area of pre- and post-acute care transitions. 
The Community and Population Health Services portfolio is accountable for HHS' Population Health, Bundled Care and Alternate Level of Care strategies. The portfolio provides system-wide support through the HHS & HNHB LHIN HCC Integrated Managers of Transitions, Site Administrative Managers, Transitional Care & Discharge Specialists, ALC Navigators, Centralized Care and Transition Team, Hospital to Home, Hamilton and Niagara North West Sub-Region Health Links, Integrated Comprehensive Care Program, and Medical Psychiatry Consultation Liaison Teams. The McMaster Family Health Team (McMaster Family Practice, Stonechurch Family Health Centre and Canadian Blood Services) and Behavioural Supports Ontario (BSO) are aligned to the portfolio.

The therapist on the Centralized Care and Transition Team (CCaTT) functions as a valuable member of the interdisciplinary team providing care to patients at high-risk for sub-optimal health outcomes and poor experience – this patient population are often older adults with multiple chronic conditions who are frail and impacted by social determinants of health. The CCaTT therapist partners with other health professionals to develop holistic care plans by assessing the physical, cognitive, environmental and social needs. The therapist practices in accordance with the standards and guidelines for professional practice as per their specific College and according to Hamilton Health Sciences policies & procedures. The therapist incorporates best available evidence into practice for example, Comprehensive Geriatric Assessment and Ontario's Assess and Restore guidelines. In addition, the Therapist will actively participate in professional practice and program committees & initiatives that support patient care, clinical practice, education and research as approved by the Clinical Manager.

12 hour shifts (days, evenings, weekends, holidays)

This role is temporary until October 2026. 

1. Current registration and in good standing with the College of Occupational Therapists of Ontario or College of Physiotherapists of Ontario 
2. Knowledge of outcome measurement and evidence-based practice
3. Demonstrated ability to work in a changing environment with high productivity, organizational and flexibility demands
with solid clinical reasoning and caseload management skills.
4. Must have excellent communication and collaboration skills, and the ability to work within interdisciplinary teams across
the hospital and into the community. 
5. Minimum of 3 years experience working with the geriatric and medicine population
6. Demonstrate knowledge, skills and abilities needed to complete cognitive, mood, functional and mobility assessments for seniors living with multiple chronic conditions and frailty. 
7. Ability to perform well in a fast-paced environment
8.Demonstrated expertise in restorative approaches
9.Demonstrated current knowledge of and experience with discharge planning and community resources
10. Ability to creatively problem solve and facilitate change.
