Duties
- Conduct comprehensive patient assessments and evaluations.
- Develop and implement individualized care plans based on patient needs.
- Administer medications and treatments as prescribed, ensuring adherence to medical protocols.
- Perform diagnostic tests and interpret results to guide treatment decisions.
- Collaborate with healthcare professionals to coordinate patient care across various settings including hospitals, nursing homes, and assisted living facilities.
- Provide education and support to patients and families regarding health management and disease prevention.
- Maintain accurate medical records using electronic health record systems such as Epic or Cerner.
- Stay current with medical terminology and best practices in nursing and patient care.
Requirements
- Valid Nurse Practitioner license with relevant certifications.
- Experience working with children, seniors, or in acute care settings is preferred.
- Familiarity with pain management techniques and case management principles.
- Proficiency in medication administration and phlebotomy skills.
- Knowledge of laboratory procedures and dementia care practices is advantageous.
- Previous experience in home care or medical office environments is a plus.
- Strong communication skills with the ability to work effectively within a multidisciplinary team.
