1.0 FTE Nurse Practitioner – Primary Care (MRP)
Health Zone Nurse Practitioner-Led Clinic (HZNPLC) is a non-profit organization funded by Ontario Health. We are a primary care clinic led by Nurse Practitioners and supported by a full interdisciplinary team including nurses, social workers, pharmacist and administrative staff. We provide care to our community in a culturally safe, inclusive and trauma-informed way, supporting both patients and providers to have a positive experience rooted in excellence.
We are currently looking to fill a permanent 1.0 FTE Nurse Practitioner position to provide excellent patient care in a busy primary care clinic.
As a Nurse Practitioner at Health Zone NPLC, you will be part of an interdisciplinary team of health and social service professionals providing high quality, accessible, and timely primary health care programs and services. You will be working to full NP scope of practice as set out by the College of Nurses of Ontario, in the role of the Most Responsible Provider. You will engage in assessment, treatment/management, health education and promotion, advocacy, and service coordination/referral with a broad range of clients who access the clinic as primary care patients.
What you'll need:
- Minimum one year of clinical experience as an NP, with preference given to candidates who have recent experience in primary care/community health, mental health, women's health, and chronic disease management
- Advanced knowledge and skills in current, evidenced-based practices in primary care delivery with an emphasis on health promotion, risk reduction and injury prevention
- Commitment to working within an inter-professional, collaborative, strengths-based, health promotion framework to address the barriers faced by marginalized populations is essential; previous experience working with marginalized populations is an asset
- Self-motivated, flexible and creative approach to work and stimulated by taking on new challenges
- Excellent interpersonal, communication, organizational, negotiation, and problem-solving skills
- Current CPR certification
- Competency in PS Suites EMR and proficient with basic computer applications
- Competency in another language is an asset
- Primary Health Care Nurse Practitioner Certificate and current registration with the College of Nurses of Ontario in the extended class is required
- Evidence of professional liability insurance and complete vaccination record is required
What we offer you:
· A salary range from $122,169.00 to $128,599.00 per annum, commensurate with experience
· Monday to Friday schedule – 8:30am – 4:30pm, some evening clinic hours until 7pm required (1-2 days per month)
