Shifts: Days (Monday to Thursday - 8:30 a.m. to 4:00 p.m.)

Rate of Pay: $34.89 - $44.47 per hour

The Social Worker is responsible for completing comprehensive psychological and social assessments for any clients that are referred for social work services. In addition to determining a client's counselling needs, the social worker may make referrals, advocate and provide education on issues related to diabetes management. This position may require travel to other communities to provide education and counselling services as well as organizing community presentations and/or support groups related to diabetes management. The social worker is part of an interdisciplinary team and acts as a consultant to other disciplines and the North Diabetes Health Network.

Qualifications:
**Education/Experience**:

- Baccalaureate degree in Social Work (BSW or HBSW)
- Current registration with the Ontario College of Social Workers and Social Service Workers
- Thorough knowledge of the Code of Ethics and Standards of Practice as set out by the Ontario College of Social Workers and Social Service Workers
- Must be a Certified Diabetes Educator designated by the Canadian Diabetes Educators Certification Board or be eligible to write and successfully pass the Certification exam
- Skills and knowledge in diabetes must be current
- Knowledge of social work values and ethics, theory and established practices of clinical social work
- Understanding of the social determinants of health of indigenous people and the social consequences and risk factors associated with client conditions (ie.addictions and mental health)
- Knowledge of family systems and family dynamic skills in working with families
- Knowledge of community systems and resources for the purposes of networking between the facility/program and other services
- Indigenous educational background directly related to social work preferred

- Ability to manage a clinical practice in a health care setting
- Ability to complete analysis, using clinical knowledge, skill and judgement to synthesize assessment information
- Ability to communicate effectively and concisely, both orally and in writing
- Ability to work independently, interdependently, and participate as a contributing member of a variety of teams within the workplace, community and ministries/agencies
- Strong time management, analytical/critical thinking, problem solving and decision making skills
- Ability to use independent judgment and maintain confidentiality of records, actions, and information

**Assets**:

- Indigenous language fluency and/or Indigenous cultural education an asset

**CONDITION OF EMPLOYMENT**
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
- Will be required to travel to various work locations and provide own transportation
