Diabetes Nurse Educator

Reporting to the Home and Community Care (HCC) Nurse Supervisor, the Diabetes Nurse Educator collaboratively develops and implements the diabetes program, addressing the physical, social, emotional, and spiritual care needs of clients and their caregivers. Diabetes Nurse Educator will work with the HCC Program. Diabetes Nurse Educator coordinates and implements in-home nursing services, works with a team of Personal Support Workers (PSW's) and other health care professionals. This individual will provide client centered nursing services to HCC clients in various KO communities. The Diabetes Nurse Educator will fulfill responsibilities in accordance with the standards of practice and guidance of the College of Nurses of Ontario.
DUTIES AND RESPONSIBILITIES- The Certified Diabetes Educator (CDE) RN/RPN will deliver services to all KO communities and provide comprehensive assessment, care planning, and education for patients and families in collaboration with the interdisciplinary team
- Participate in the development and implementation of the diabetes program and standards of care with the HCC Nurse Supervisor. Coordinates and implements in-home nursing services under the direction of the HCC Nurse Supervisor, and in conjunction with KO community Nurse in Charge and community Health Director.
- Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring blood glucose levels.
- Partner with clients to assist with decisions regarding their diabetes management and support and educate clients on health lifestyle choices.
- Provide ongoing assessment and recommendations to the treatment team and patient/family/community support systems for adjustments in plan of care as required.
- Report to the HCC Nurse Supervisor on all matters pertaining to the Diabetes Education Program.
- Perform other related duties as requested by HCC Nurse Supervisor, KO Health Director and KO DON
- Charting using the Community Electronic Medical Record (cEMR) Mustimuhw
- A Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
- Training/certification specific to diabetes education and management or intent to obtain certification
- Must have BLS or must be willing to obtain.
- Knowledge on culture and tradition of remote Indigenous communities.
- Excellent verbal and written communication skills. Ability to speak Ojibway, Oji-Cree, or Cree an asset.
- Commitment to ongoing learning and development of professionalism.
- Awareness of working with unregulated Health Workers; Transfer/Delegation of Skills, processes, and requirements.
- Ability to work independently or as a proactive team member.
- Must have demonstrated ability to work with the Indigenous population as well as health and social service agencies and understanding cultural norms, protocols, and diversities.
- Full time employment with the flexibility of weekends if required. Dependent on situation and any crisis situations. May be required to fly into the KO Communities. Please note this position will be assessed yearly based on funding.
