**Who we need**

We are seeking a compassionate and professional **Social Worker** to join our team and provide clinical guidance and therapeutic interventions for infants, children, youth, and their families. As a Social Worker, you will be responsible for coordinating the overall treatment plan in a multidisciplinary context, delivering evidence-based and evidence-informed individual, family, and group counselling, and ensuring that services are provided in a coordinated and timely fashion.

**Responsibilities**:

- Provide a written comprehensive psycho-social assessment, develop a clinical formulation, and create a shared multi-faceted treatment plan in collaboration with the multidisciplinary team and in partnership with clients.
- Gather and review assessments and other information from other resources to inform treatment planning.
- Actively guide and participate in the assessment and treatment planning process with clients, monitor progress toward goals, and modify the treatment plan as indicated.
- Develop and implement treatment plans and ensure that service, including intake/assessment, transition and discharge planning, is delivered in a coordinated and timely fashion.
- Collaborate with a multidisciplinary team and involve consultation as appropriate to inform assessment, treatment planning, and implementation.
- Provide case management services, including initiating and participating in case conferences and treatment reviews.
- Recommend and refer clients to appropriate community mental health treatment and support services.
- Liaise, consult, and coordinate with required professionals (internal and external) and community services to ensure effective clinical treatment.
- Maintain agency records as required, including assessment reports, intervention plans, closing reports, pre/post measures, case notes, statistics, and other documentation, as required by agency standards.
- Administer child and parent measures at prescribed intervals and submit data for analysis to inform treatment planning.
- Ensure internal policies and procedures are in adherence.
- Participate in developing training programs and materials for other professionals and community groups.

**Qualifications and Skills**:

- Bachelor’s Degree in Social Work.
- Registration with a professional college (OCSWSSW, CRPO) qualified to provide psychotherapeutic interventions.
- 1-2 years of experience in children’s mental health.
- Strong clinical skills in assessment, formulation, and treatment with an ability to utilize flexible strategies for engaging and working with families, individuals, groups, and larger systems.
- Strong clinical knowledge and experience in relevant theoretical approaches (e.g. trauma-informed treatment, attachment, CBT, TF-CBT, etc.).
- Proficient with MS Office Suite and navigating database systems.
- Possess strong verbal and written communication skills.
- Demonstrated ability to work in a respectful manner with groups from diverse backgrounds and experiences.

**Compensation**: $66,731 per annum
