**Position Overview**:
**Key Responsibilities**:

- Participate in the resident admission process, including tours, admissions, transfers, discharges, and occupancy tracking.
- Act as a liaison with families and advocate for residents to ensure their rights are respected and upheld.
- Collaborate with the interdisciplinary team in resident assessment, care planning, and evaluation to deliver personalized care.
- Provide therapeutic services through individual and group counseling for residents and families, documenting and evaluating the effectiveness of these programs.
- Adhere to established department plans, policies, procedures, and quality assurance programs to meet regulatory and Extendicare standards.
- Deliver in-service programs for staff, residents, and families, and support professional development initiatives.
- Maintain accurate records, reports, statistics, and documentation in compliance with policies and legislative requirements.
- Participate in committees related to resident care and facility operations as required.
- Consult with community partners, company resources, and professionals to address resident care needs and department activities.
- Recognize and refer cases beyond the scope of practice to a qualified Social Worker.

**Qualifications**:

- Diploma in Social Service Work from an accredited college (mandatory).
- Registration with the Canadian Association of Social Workers and/or Ontario College of Social Workers (mandatory).
- Previous experience in a long-term care setting is preferred.
- Strong clinical skills in assessments, care planning, supportive therapy, crisis intervention, and grief counseling (preferred).
- Expertise in working with geriatric populations, including Alzheimer’s disease, dementia, depression, responsive behaviors, and other psychogeriatric issues (preferred).
- Knowledge of applicable Ministry of Health and Long-Term Care legislation and standards (preferred).
- Familiarity with occupational health and safety practices, principles, and legislation (preferred).
- Excellent communication, organizational, and team collaboration skills.
