The West Toronto Ontario Health Team (WTOHT) is seeking a Social Worker for a full-time position beginning with a three-month contract with the possibility for a 12-month contract extension and annual renewal.
The Social Worker - Supported Intake will work directly with West Toronto residents seeking attachment to a primary care provider. Working with WTOHT's Supported Intake team, this team member will ensure client care goals are well-documented to achieve lasting attachments to primary care.
Key competencies and requirements include:
- Assessment experience including recording client histories;
- Unwavering focus on the client experience;
- Knowledge of community resources;
- Resourcefulness to locate community resources;
- Communication and teamwork to support transitions in care; and
- Current registration or in process of securing registration with the Ontario College of Social Workers and Social Service Workers
Role will primarily work from home with equipment (phone, laptop) supplied to the successful candidate
Start Date: As soon as possible
Wage Rate: $39.05 per hour over a 37.5 work week
Schedule: Please indicate your preference for a 8 AM to 4 PM or 12 PM to 8 PM shift in your reply.
