Pathways Assertive Outreach
**Note**:This is a contract 22.5hr/wk position with our Pathways Assertive Outreach team in Housing to March 31/25.
Reporting to the Clinical Team Lead, the Program Coordinator plays a pivotal role in facilitating comprehensive care for individuals within the program. The Program Coordinator ensures that participants receive appropriate support and resources to meet their unique needs and improve their overall well-being.
What you will do:
Program Philosophy
- Offer culturally supportive services where indicated
- Uphold the principles of the ACT Model, Harm Reduction and Trauma Informed Care
- Understand and follow program policies and procedures
Administrative
- Complete data collection and entry
- Complete daily schedule
- Schedule, coordinate and facilitate care conferences
- Manage the team inbox including communication with partner programs about participant care
Community Treatment Order Responsibilities
- Provide administrative support to Clinical Team Leads for all CTO-related matters
- Input, track and update CTO records as needed
- Maintain positive relationship and communicate directly with AHS CTO officials as needed
- Provide administrative support for apprehension process as needed
- Gather necessary signatures and submit documents as appropriate
Participant Intake
- Serve as the primary point of contact for individuals or system partners seeking services, responding to inquiries, providing information about available programs and services, and supporting to navigate the referral process
- Conduct initial screenings or assessments to gather relevant demographic, clinical, and insurance information from participants
- Participate in the team intake and triage meetings
- Coordinate intake appointments, communication, and administrative tasks once admitted to program
- Assess participant needs and determine eligibility for services based on established criteria and program requirements
Service Coordination and Referrals
- Connect participants with healthcare services, including medical specialists, primary care physicians, mental health providers, and rehabilitation services.
- Facilitate referrals to community resources such as housing assistance programs, transportation services, and financial assistance programs.
- Coordinate appointments and follow-up care to ensure continuity of services and address any barriers to access
- Coordinate internal referrals, including, but not limited to completing intake support
- Collaborate with program participants, families, hospitals, pharmacies, and partner agencies to assist participants in achieving treatment and recovery goals while maintaining continuity of care
- Nurture professional relationships with other service providers to ensure a high quality of service for our participants
Documentation and Reporting
- Maintain accurate and up-to-date documentation of client assessments, care plans, and service coordination activities in electronic health records or case management systems
- Ensure confidentiality and privacy of participant information in accordance with legal and ethical standards
- Maintain accurate and up-to-date records of participant intake information, including demographic data, assessment results, and referral details
- Manage and track completion of participant documents
- Enter intake data into electronic health records according to established protocols
Team
- Participate in team meetings, care conferences, or trainings to enhance collaboration and knowledge sharing among team members
- Demonstrate initiative and an ability to work as an influential member of a team under mínimal supervision, supporting colleagues as needed
- Display a high degree of professionalism as well as collaborate with partner agencies
- Welcome and provide ongoing feedback to promote a healthy workplace environment, strong team cohesion and exceptional program delivery
- Contribute positively to team culture

What you bring:
- Within one year of completing a Bachelor’s degree or diploma in a relevant field is preferred
- Minimum two years of experience in a social service setting or within an interprofessional care team model
- Strong knowledge base of local resources for housing, mental health, addiction, community support, finances, etc.
- Extensive knowledge of Alberta Mental Health Act and Community Treatment Orders
- Exceptional written and oral communication skills, including active listening skills
- Exceptional organizational skills
- Commitment to contributing positively to team culture
- Openness to a wide scope of practice with a commitment to innovation and flexibility
- Strong initiative in times of limited supervision
- Basic First Aid - Level C CPR within 30 days of hire date, certification must be valid for a minimum of 6 months upon hire
- A satisfactory vulnerable sector criminal record clearance is required
