**The Opportunity**:
Reporting to the Program Manager, the Program Coordinator is responsible for the administration and day-to-day operations of the program. Responsibilities include all aspects of the supervision/direction of the rehabilitation team, ensuring compliance with Agency standards and facilitating the development of individuals within a 24-hour wholistic model.
**A typical day as a Program Coordinator may include**:
- Builds strong individual relations, delivering individual-centric solutions and identifying opportunities that benefit the individuals in the program.
- Lead a team of Community Rehabilitation Workers, ensuring that the individual’s goals focusing on quality of life are implemented and an environment with high standards is maintained.
- Build a positive team environment and ensure that staff keep current with required trainings and certifications.
- With support from Program Manager, coordinates program intakes to ensure occupancy remains high and individuals are matched with the program that will best meet their needs.
- Plan, schedule, and attend annual individual conferences and ensuring staff provide timely updates about status of individual goals.
- Ensures that all necessary intake forms are completed, and consents are updated on a yearly basis as per accreditation standards.
- Supports the team to implement activities and outings that focus on quality of life and skills for the individuals.
- Communicates and consults with families and guardians on a regular basis, providing solutions and support to address their concerns and feedback.
- Keep current on all individual health concerns, ensuring proper procedures are followed by team members and identifying when additional consultations need to occur with external health care providers.
- Ensures all recommendations for changes/improvements to dietary issues/therapeutic feeding techniques, etc. are forwarded to the appropriate specialist.
- Maintains confidentiality, adhering to the Ethical Code of Conduct and monitors program operations to ensure compliance to all policies, procedures, legislation, accreditation and Health and Safety standards.
- Accurately completing all paperwork requirements related to individuals and agency policy, procedures and processes.
- Completes annual planning of individual budgets as appropriate and monitoring all site/individual funds.
- Ensures that all monthly paperwork related to the program, expenses and timesheets are accurate and submitted on-time.
- Participates in an on-call rotation.
- Chairs program meetings and attends all other regular scheduled meetings.
- Facilitates training for team members within URSA as needed.
**What you offer**:
- A certificate, diploma, or degree in rehabilitation or related human services field
- 3-5 years’ experience working with persons with disabilities
- Minimum 3 years’ experience in administration and supervision of a 7-20-member team.
- Willing to work a flexible schedule to support the needs of the program and organization
- Ability to coordinate services within a multi disciplinary team
- Experience working with individuals with cognitive disability
- Have current Standard First Aid and level C CPR
- Clear Criminal Record Check with Vulnerable Sector Check
- Clear Child Intervention Record Check
- A valid Class 5 driver’s license with 3 years clear driving record in Canada and must become an agency van driver.
- A minimum of $1,000,000 in liability insurance
