As a leading employer that operates residences and supports our mission, of the Journey program, “To empower and sustain a cultural shift that provides seniors with dementia greater involvement in all aspects of their life,” is now looking for a passionate and driven individual to assist with provision of Person-Centered Care to residents with dementia and/or other neurocognitive disorders.
Key Responsibilities:
- A full-time designated professional who serves as the Journey Engagement Manager.
- Overseeing the day-to-day operations to include financial cost efficiencies and provision of quality care and quality of life services to residents in the program.
- Collaboration with senior leadership, the programs physician who oversees the care, treatment, and services provided to individuals to include a priority focus on the monitoring the use of psychotropic medications in order to minimize misuse or overuse of these medications.
- Assist in creating a vision and mission document for the program and share this document with all leaders, team mates, residents and families
- Model Person Centered Care by knocking on resident’s doors before entering, always making eye contact, demonstrating known knowledge about the resident during conversations.
- Incorporate Person Centre language to show respect and dignity when interacting with others and continually monitor and correct, in a respectful manner, when the use of inappropriate language is used.
- Support team mates to further their education and skills
- Include teammates in decision-making and provide opportunities for them to give feedback or express their concerns.
- Encourage team mates to try new approaches and offer suggestions that will enhance resident’s quality of life
- Integration of care and services that are person-centered and incorporates the personal preferences, values, lifestyle choices and needs of the resident
- Determining the stage of dementia (Mild, Moderate or Severe), the resident’s cognitive and physical functional assessments, service care planning, medication management, behavioral interventions, and therapy for activities of daily living needs.
- Develops an individualized resident engagement calendar that offers structured and personalized activities that will assist in reducing agitation, improving mood and enhance the quality of life for the resident.
- Provide a safe and comfortable environment that is supportive of the residents needs to maintain independence; to incorporate environmental supports in an unobtrusive manner, which encourages good nutrition, physical fitness, continuing meaningful roles and responsibilities and social connections.
- Develop and maintain a quality committee that will focus on meeting these standards and base their evaluation of the 7 key Core Elements of the Journey Care Program using the 10 standards.
Education & Experience:
- Minimum 3 years’ experience in Geriatrics, Rehabilitation and/or Retirement industry.
- Effective interpersonal and communication skills to establish and maintain strong relationships with internal and external stakeholders.
- GPA, Ufirst or PIECES training preferred
- Must possess excellent supervisory and leadership abilities.
- Must express initiative, good judgment and foster a positive work atmosphere.
- Strong computer skills in Microsoft Office, Excel, Outlook.
- Strong analytical, problem solving and decision-making skills.
- Ability to work effectively in a team, independently and under stressful conditions.
- Effective time management skills and the ability to manage multiple priorities.
