**POSITION**: Aging Out of Care Program Coordinator
**LOCATION**: Little Red Youth Wellness Centre
Montreal Lake Child and Family Agency
**SALARY RANGE**:$62,400 - $84,100 (Based on experience and qualification)
**SUPERVISOR**: Program Manager
**HOURS**:Monday through Friday9:00am
- 5:00pm
**ROLE**:Facilitates engagement and comprehensive case management for youth as they age out of care. Through outreach and family engaged case management the Aging Out of Care Program Coordinator will assess and determine the needs of the youth. This role involves strong advocacy and the ability to navigate through multiple systems to reach appropriate resource allocation. Working knowledge of available resources and accessibility of these resources will be a fundamental element of this role. This includes assisting and connecting with community, personal and provincial support resources and building on informal support systems.

**DUTIES AND RESPONSIBILITIES**:
- Receive and review referrals in a timely fashion
- Identify equitable standards that are required for the youth
- Develop and implement comprehensive case management for all referrals
- Establish a strong working rapport with the youth and families
- Supervise and mentor the Aging Out of Care Program Outreach Worker
- Provide monthly and ongoing reports for the Executive Director and Board of Directors of the Aging Out Program
- Outreach and engage with youth to achieve case management goals
- Maintain client confidence and protect operations by adhering to strict confidentiality
- Engage with other departments and programs to ensure appropriate connection of required resources
- Establish a compassionate and comfortable environment for the youth, families, and support networks
- Maintain accurate and up-to-date case notes of each working file and perform follow-up tasks on a regular basis
- Organize, assign and oversee tasks and duties delegated to the Aging Out Program Outreach Worker
- Maintain professional working knowledge of the position and attend training that is directly applicable to the duties of the Aging Out Program Coordinator
- Travelling will be required from time to time to perform the duties of this position
- Adhere to the Montreal Lake Child and Family Agency Code of Ethics
**KEY ACTIVITIES**:
A. Comprehensive Case Management
- Complete a thorough intake with assessment of all equitable standards of care (Financial, Educational and Professional Development, Housing, Relationships, Culture and Spirituality, Health and Well-being, Advocacy and Rights, and Emerging Adulthood Development
- Connect to other personal, community and provincial resources as needed
- Engage families and caregivers when required
- Complete timely and routine follow up on case management objectives
B. Outreach Services
- Perform outreach support with Aging Out Program Outreach Worker when required
- Travel with the youth to various appointments or activities as indicated in the case management plan
- Liaison with existing community services when required
- Commence an information campaign with the Aging Out Program Outreach Worker to ensure all community members and youth are aware of services offered
C. Clerical
- Prepares reports, and records statistics
- Maintains client case notes, update comprehensive case management plans and other information as required
- Assist Aging Out Program Outreach Worker with repots and clerical duties as required
**QUALIFICATIONS**:
- Bachelor’s Degree in social work, Psychology or Human Service Field or equivalent with 2 years of experience working with children and families
- Current CPR Level C
- Experience working in a multidisciplinary team environment
- Experience working with complex clients
- Experience and demonstrated empathy working with marginalized populations
- Ability to establish and maintain effective working relationships with a wide variety of professionals and members of the public
- Ability to initiate and exercise critical thinking and decision making
- Demonstrated empathy, trust and respect in all aspects of the job
- The ability to understand the Woodland Cree language and have extensive knowledge of First Nations culture, practices, protocols, customs and belief systems is a definite asset
- Driver’s license and access to a reliable vehicle
- Must be willing to work evenings and weekends when required
- Ability to work autonomously and collaborate with the team when required.
