**SALARY RANGE**:$57,135.00 -$77,779.44

**SUPERVISOR**: Program Manager

**HOURS**:Monday through Friday9:00am
- 5:00pm

**ROLE**:Facilitates engagement and comprehensive case management for youth as they age out of care under the direct supervision of the Aging Out of Care Program Coordinator. This is a community-based position and will involve outreach to youth and families who require this service. This role involves strong advocacy and the ability to navigate through multiple systems to reach appropriate resource allocation. This includes assisting and connecting with community, personal and provincial support resources and building on informal support systems. Strong clerical and interpersonal skills will be required to ensure seamless program delivery.

**DUTIES AND RESPONSIBILITIES**:

- Develop and implement comprehensive case management for all referrals
- Establish a strong working rapport with the youth and families
- Outreach and engage with youth to achieve case management goals
- Maintain client confidence and protect operations by adhering to strict confidentiality
- Engage with other departments and programs to ensure appropriate connection of required resources
- Establish a compassionate and comfortable environment for the youth, families, and support networks
- Request direction and supervision when needed
- Interact with youth and families in a professional manner
- Maintain accurate and up-to-date case notes of each working file and perform follow-up tasks on a regular basis
- Maintain professional working knowledge of the position and attend training that is directly applicable to the duties of the Aging Out Program Coordinator Program Outreach Worker
- Travelling will be required from time to time to perform the duties of this position
- Adhere to the Montreal Lake Child and Family Agency Code of Ethics
- Perform wellness checks, including home visits, within the community with the Aging Out of Care Program Coordinator
- Report any incidents to supervisor

**KEY ACTIVITIES**:
A. Assist with Comprehensive Case Management
- Identify referral forms and ways to connect with other personal, community and provincial resources as needed
- Engage families and caregivers when required
- Complete timely and routine follow up on case management objectives as advised by the Aging Out of Care Program Coordinator
- Maintain correct contact information for ease of follow up

B. Outreach Services
- Perform outreach support with Aging Out of Care Program Coordinator when required
- Travel with the youth to various appointments or activities as indicated in the case management plan
- Liaison with existing community services when required
- Assist in the delivery of an information campaign with the Aging Out of Care Program Coordinator to ensure all community members and youth are aware of services offered

C. Clerical
- Prepares reports and statistics while keep tidy and up to date files
- Updates demographics and engagement case notes for each youth.
- Assist Aging Out of Care Program Coordinator with repots and clerical duties as required.

**QUALIFICATIONS**:

- Grade 12 or equivalent is required. Additional post secondary education would be preferred but not required.
- Relevant experience with youth and families is considered an asset
- Current CPR Level C
- Experience working in a multidisciplinary team environment
- Experience working with complex clients
- Experience and demonstrated empathy working with marginalized populations Ability to establish and maintain effective working relationships with a wide variety of professionals and members of the public
- Ability to initiate and exercise critical thinking and decision making
- Demonstrated empathy, trust and respect in all aspects of the job
- The ability to understand the Woodland Cree language and have extensive knowledge of First Nations culture, practices, protocols, customs and belief systems is a definite asset
- Driver’s license and access to a reliable vehicle
- Must be willing to work evenings and weekends when required
- Ability to work autonomously and collaborate with the team when required.

- Excellent interpersonal and communication skills as well as written, organizational and time management skills
