Case Manager, Tft

**QUALIFICATIONS**: - Bachelor's degree in a healthcare related field with active registration/membership in a health or health related professional college - Must be a member in good standing of a respective regulatory College (COTO, CNO, OCSWSSW) under the Regulated Health Profession Act, 1991 - Current license/registration in related field - Minimum of two years experience in Clinical Hospital environment - Minimum two years of recent experience in case management, utilization management, discharge planning - Understanding of Alternate Level of Care (ALC,) patient flow processes - Experience in collaborative care planning within hospital and community setting - Demonstrated knowledge of medical terminology, ability to understand patient care plans, pathways and treatment plans - Demonstrated knowledge of coordinating and leading discharge planning with the interdisciplinary care teams and patients and their families - Demonstrated understanding of financial and personal care capacity, and the ability to consider capacity for the purposes of Discharge Planning - Comprehensive and demonstrated knowledge of community resources including congregate care, housing, community supports services, and social service agencies - Evidence of effective organizational, interpersonal, communication, leadership, judgment and decision-making skills - Applies Niagara Health values and professional practice standards to patient/client care and service - Demonstrated commitment to patient/client centred care - Demonstrated commitment to collaborative planning and relationship building - Applies skills by providing, facilitating and promoting quality patient care within an inter-professional team - Demonstrated commitment to patient and family education and the ability to promote health and wellness - Demonstrated commitment to patient safety and safe transitions in care - Integrates education, critical thinking, and relevant practical experience in problem solving - Consistent and regular attendance mandatory - Proficiency in both official languages (English and French) will be considered an asset - We are especially interested in connecting if you..._ - Value diversity, equity and inclusion;_ - Are excited about being part of a respectful and hard-working team in the beautiful Region of Niagara;_ - Identify as a member of one or more of the following equity-deserving groups: Indigenous persons, racialized, persons with disabilities, persons of diverse sexual orientations and gender identities and all others who may contribute to the further diversification of ideas._ - Have background or expertise in the healthcare environment, supporting patient care and providing extraordinary customer-service._ - In accordance with the Accessibility for Ontarians with Disabilities Act, 2005 and the Ontario Human Rights Code, Niagara Health will provide accommodations through the recruitment process to applicants with disabilities.
