**About the Dufferin HART Hub**
The Dufferin HART Hub is dedicated to improving outcomes for individuals in Dufferin County experiencing homelessness, mental health challenges, and substance use issues. By providing integrated, low-barrier support services, we enhance the quality of life, physical health, and overall well-being of individuals in our community. We take a person-centered, equity-focused approach to ensure that clients have access to the care they need when they need it.
**Role Overview**
A dynamic and compassionate Community Mental Health Counsellor (CMHC) whom is an integral part of the HART Hub Dufferin program. The CMHC offers strengths-based case management from a recovery perspective to support clients of the program to find and maintain safe and affordable housing using a housing first approach. They are a resource and an advocate in their comprehensive understanding of crisis management and the impact mental health challenges might have on an individual and their overall wellbeing. Their main goal is to ensure clients receive the best support possible to enable them to access and achieve independence in their home and daily activities.
**Duties and Responsibilities**:
- Liaise with the intake and HART Hub team to support successful transition into supportive housing using a housing first approach
- Support clients’ transition into supportive housing and promote housing stability
- Develop and implement individualized wellness, crisis, and recovery plans
- Conduct assessments to enhance clients’ mental and physical health and community integration
- Provide strengths-based case management and assist with developing daily skills
- Facilitate access to holistic, person-centred services addressing social determinants of health
- Partner with clients, families, and other health care team members
- Maintain a network of community resources and coordinate referrals
- Work collaboratively and independently within a multidisciplinary team
- Maintain accurate records and statistical data in line with agency standards
- Uphold agency health and safety and client safety protocols
- Promote inclusive, equitable, anti-racist, and anti-oppressive practices
- Comply with PHIPA legislation and demonstrate strong communication and organizational skills
- Perform additional duties as required/delegated by Management
**Qualifications**:
- Post-secondary education in Social Work, Health Services, Psychology or equivalent
- 3 years providing Mental Health case management or equivalent care
- Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with assessment
- Experience working within the homelessness sector
- Knowledge of evidence-based practice for mental health services
- Excellent assessment, risk management, and crisis intervention skills
- Expertise in collaboration with healthcare services
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
- A vehicle, valid driver’s license, and adequate insurance coverage
- Valid Certification in First Aid & CPR
- Training in ASIST, CPI and Naloxone administration
**Reporting Function**:
The Community Mental Health Counsellor reports directly to the Program Manager. They work with both internal and external partners and interact with all levels within the organization. They are a support system, resource, and an advocator for the team and organization.
**Working Conditions and Requirements**:
- Hours are typically Monday - Friday, 8:30am - 4:30pm but may vary depending on client needs
- Able to sit/stand for prolonged periods of time (building inspections, site meetings, unit inspections, etc.)
- Able to answer and communicate telephone calls and general computer use
- Able to lift up to 20lbs (product, devices etc.)
- Vehicle with adequate insurance coverage to travel to program sites and throughout the community is required
