We take a person‑centered, equity‑focused approach to ensure that clients have access to the care they need when they need it.Role OverviewFamily Transition Place (FTP) is seeking a dynamic and compassionate Mental Health Counsellor to be an integral part of the HART Hub Dufferin collaborating with a multidisciplinary team delivering comprehensive, integrated care services to vulnerable individuals. The Mental Health Counsellor will provide brief mental health case management support and deliver time‑limited, client‑centered interventions. They will use evidence‑based approaches to monitor these interventions and work closely with a variety of professionals. This position will receive direction from the HART Hub Manager for day‑to‑day operations and is accountable to the FTP Program Supervisor. The Mental Health Counsellor will work with both internal and external partners and will interact at all levels within the organization.ResponsibilitiesCollaborate in developing and implementing individualized wellness plans with clientsUse standardized assessment tools designed to improve and/or stabilize client’s physical and mental healthOffer a strength‑based approach that recognizes client’s unique needsAssist with activities of daily living, conflict resolution, and crisis avoidanceProvide comprehensive, holistic, and person/family centered access to services that address the social determinants of healthPartner with clients and their families as members of the health care teamDevelop and maintain a community resource networkRefer to partnering organizations and provide linkages to community supportsWork independently and collaboratively within a multidisciplinary teamUnderstand and be able to integrate a philosophy of person and family centered care into daily responsibilitiesDemonstrate a strong commitment to quality care and service delivery based on the HART Hub Mission and ValuesEnsure appropriate records are kept and ensure that statistical information is maintained as per agency standardsAdhering to FTP and HART Hub policies and procedures and compliance with regulatory requirements such as OHSA, AODA, etc.Perform additional duties as required/delegated by ManagementQualificationsPost‑secondary education in Social Work, Health Services, Psychology or equivalent.Minimum 2 years of experience in the mental health and addictions sectors with a focus on assessment, brief interventions and short term supports.Demonstrated knowledge of serious mental illness, addictions, concurrent disorders, homelessness, and experience with assessment.Experience working within the homelessness sector.Strong written and oral communication skills.Employment RequirementsVulnerable Sector Police Clearance Certificate and maintaining of clearance requirements during employment.Valid driver’s license and insurance.CPR and First Aid CertificationWorking Conditions and Physical Requirements35 hours per week, including day, afternoon and weekend hours to be established.Able to work well with both internal and external clients.Travel by vehicle to community sites for client support as required.Compensation$28.60 to $31.77 per hour + allowance in lieu of benefitsThe HART Hub Dufferin is at the forefront of providing integrated care for individuals facing complex challenges in Dufferin County.
