Edmonton Integrated Services (EIS) is currently seeking an individual to fill the position of **Monitor** (Community Disability Service Practitioner) in the **Supported Living Program**.

**Position Summary**: The Monitor’s primary responsibility is the day-to-day monitoring of residential and outreach supports.

**Duties and Responsibilities**:

- To monitor homes on a regular basis and ensure Health and Safety Protocols are being followed
- Participate with the individual and the individuals support network to develop and document individualized, person centered plans
- Facilitate the implementation of support plans
- To ensure and monitor the supports identified in service proposals are being provided
- To ensure and monitor compliance to Agency “compliance” requirements
- Maintain clear, objective records; complete required reports and other documentation
- Ensure that support / service providers are aware of Agency Policy and Procedures and CET standards as they relate to them

**You will bring**:
**Formal Education**: Post-Secondary diploma/degree in Community Rehabilitation, Disability Studies, or a related field.

**Experience**: Minimum 3 years’ experience supporting individuals in a community setting; some supervisory experience an asset. Crisis Prevention and Intervention experience required.

**Knowledge, Skills, Abilities**:

- A positive attitude and belief that persons with disabilities have the right and responsibility to define and pursue their life choices, and have active involvement in their community
- An understanding of the opportunities and options available to persons with disabilities
- Demonstrated ability to work effectively in a team
- oriented work environment as well as being able to work independently with minimum supervision
- Excellent communication skills
- Must be able to identify issues, set priorities, and implement an appropriate plan of action
- Well-developed reporting and computer skills
- Effective time-management and organizational skills

**Other**:

- Must have a reliable car with personal liability insurance with a minimum coverage of two million ($2,000,000.00)
