Registered Practical Nurse (RPN)

CareKW is recruiting RPNs to provide support to patients at Transitional Care Unit.

The transitional care unit is a care facility designed to support complex patients transitioning from the hospital/home environment to long-term care. The CareKW team works to provide the best experience for patients during their stay in the TCU.

CareKW is focused on creating a team of qualified individuals to enhance the health of the community and our clients. We are open to Nurses with a variety of backgrounds and training expertise.

**Registered Practical Nurse Role**:

- Completing client intake visits and any follow up visits as needed
- Conduct assessments and implement a plan of care
- Assisting in skills training as needed
- Provide direct skilled nursing care and monitoring of patients
- Conduct direct communication with patients and/or clients, families, and other team members to provide optimal health care
- Develop thoroughly written reports, reporting to the Director of Nursing and Operations Team coordinators
- Medication oversight and administration
- Practice within the scope of practice set out by the College of Nurses of Ontario (CNO)

**Registered Practical Nurse Role Requirements**:
Graduate of an accredited Registered/Licensed Practical Nurse program-
Familiar with equipment including monitors, IV pumps, ventilators, feeding tubes, central venous line management, etcTracheostomy and ventilation care experience is considered an asset
- VAC Dressing / Negative Pressure Wound Therapy training/experience considered an assetExperience in community nursing is considered an assetValid work/study permit (if applicable)Current CPR/First Aid CertificationComplete immunization record (MMR, TDAP, Varicella, Hep B)Clean Vulnerable Sector Screen (within 3 months of hire)Strong documentation and clinical skillsAbility to work with little supervisionHigh level of interpersonal skillsAccess to a vehicle is considered an asset
