**Position Summary**:
The Mental Health & Wellness Navigator, under the direction of the STC Health Centre Manger, will be primarily responsible for assessing client needs and support systems and assist clients to reach their identified health goals, acting as a resource to navigate social service and healthcare systems and ensuring that they have the supports they need to optimize their health and wellbeing. The Mental Health & Wellness Navigator will work to improve access to mental health and addiction services for clients, improve health outcomes by removing barriers to access, promote early intervention that will mitigate the need for higher intensity support later on and reduce health disparities by ensuring access to services that are appropriate and available where and when clients need them.

**Responsibilities**:

- Establishes and maintains knowledge of community resources and builds strong relationships with external agencies and partners;
- Works collaboratively with community partners to reduce barriers and develop collaborative approaches to ensure accessible mental health and addiction services;
- Provide mental health assessments, supportive counseling, referrals, and intervention services;
- Provide crisis intervention support;
- Provide referrals to appropriate agencies and services;
- Provide education on risk prevention to patients, their families and support groups;
- Act as an advocate on behalf of clients;
- Collaborate with health professionals where appropriate through consultation and case conferences to evaluate progress and to assess client’s mental health;
- Assist the care team with developing individual care plans and implement and monitor those plans;
- Conduct workshops and presentations on a variety of mental health related topics;
- Complete all documentation in a professional and timely manner to ensure a high standard of care, quality assurance and legal requirements;
- Maintain accurate records and documentation in accordance with evaluation and funding agreement standards and best practice standards;
- Maintain high level of confidentiality standards;
- Promote cultural considerations with clients as appropriate;
- Demonstrate behaviours and attitude that align with STC strategic priorities and values as a place that is judgement free, accepting, compassionate and empathetic regardless of race, nationality, or economic standing;
- Practice within the limitations defined in the member’s scope of practice and assume responsibility for personal and professional development;
- Work collaboratively with team members on program development and evaluation;
- Collect project data and complete program reports as required;
- Attend and participate in project staff meetings;
- Participate in STC Accreditation and maintain best practices and quality improvement;
- Adhere to STC policies and procedures;
- Perform other related duties as defined and assigned on an as and when required basis.

**Education and Experience**:

- Post-secondary education in a related field of social services, social work, addictions, mental health, community development etc;
- Two (2) years experience working with individuals living with mental health and addictions;
- Strong interpersonal, counseling and advocacy skills;
- Experience working effectively with culturally, economically and socially diverse clients;
- Familiarity with social service and health programs, services and systems in Saskatoon;
- Expertise in working with clients with mental health and addiction issues;
- Demonstrated ability to work independently and as an effective member of an interdisciplinary team;
- Excellent English written and oral communication skills;
- Experience working with individuals with mental health and addiction issues;
- Highly self-motivated, energetic, organized, and detail oriented with an attitude for continuous improvement;
- Must possess a valid Saskatchewan driver’s license, a reliable vehicle and meet STC’s insurance requirements;
- Current CPR-HCP/BLS certification is an asset;
- Knowledge of the political, social and economic objectives of the Saskatoon Tribal Council and of the Dakota, Cree, and Saulteaux cultures will be considered an asset.

**Knowledge, Skills and/or Abilities**:

- Demonstrated success connecting and networking with vulnerable populations;
- A demonstrated ability to meet and work with clients, utilizing sound judgment, tact, initiative and high standards of integrity and responsibility;
- Understanding of harm reduction strategies;
- Understanding of Trauma and Violence Informed Care
- Understanding of the determinants of health;
- Knowledge of community resources;
- Competency in one of STC’s First Nations languages would be an asset (Cree, Salteaux, or Dakota);
- Knowledge and expertise with de-escalation techniques;
- Demonstrate respect while upholding STC’s internal and external partnerships;
