**PERSONAL SUPPORT WORKER**

**Location: East End (Sagamok Anishnawbek & Atikamesksheng Anishnawbek)**

**Casual/Relief Part-Time**

**Starting Salary: $23.00/hour (commensurate with experience)**

**Who we are?** We are a leader in First Nation Health. Our foundation is Indigenous Health in Indigenous Hands. We provide accessible, quality, culturally safe and holistic health care that supports and enhances the wellness of individuals, families, and communities. We work with our 11 First Nation community partners in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services and health promotion services. We have recently been approved as the first Indigenous lead Ontario Health Team by the Ministry of Health.

**The opportunity**: Under the direction of the Director of Home and Community Support Services, and the clinical supervision from the Home Care Service Manager for the Assisted Living Services, this position is responsible to ensure a continuum of care for vulnerable, high-risk seniors as per the care plan.

**What you need to bring with you**:

- Personal Support Worker Certificate from a recognized accredited program; with relevant experience in working in a community-based and home care setting is an asset.
- Problem-solving, good judgement and decision-making skill and ability;
- Ability to prioritize professional duties, manage multiple patients, and efficiently organize workload;
- Strong interpersonal and collaboration skills;
- Experience in electronic charting is an asset;
- Ability to work independently and within a multi-disciplinary team;
- Must possess a valid Ontario Driver’s License and a reliable vehicle, and willing to provide a driver’s abstract.

**What to expect?** Given the traditional practices of Indigenous people, from time-to-time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage, or cedar may occur.
