**Function Summary**
Reporting to the Director, Clinical Programs - Transitional Care, this role is responsible for the triage and coordination of a high-quality outpatient program for Bruyère. This triages referrals and coordinates access for patients to outpatient bundled care programs offered by a Home Care Provider agency. This role maintains the appropriate flow of patients to the program and coordinates the delivery of rehabilitation therapies. This role collaborates with the Director, Clinical Programs - Transitional Care and Home Care Provider to ensure smooth transitions of care, reporting and evaluation as required by Ontario Health.

This role is also responsible for:

- Day to day patient coordination and triage
- Chart reviews
- Patient triage documentation
- Adherence to Bruyère polices and procedures
- Clinic systems management
- Navigating patients through the outpatient continuum of care
- Acting as a resource to staff, physicians and patients
- Liaising with Ontario Health, Home and Community Care Support Services, Home Care Provider and other health care partners
- Managing patient complaints with triage and coordination services

**Required Qualifications**
- Bachelor’s degree in Nursing, Social Work or other applicable Allied Health Professional
- Member of a health professional college regulated under the Regulated Health Professions Act of Ontario
- Minimum five (5) years’ experience in marketing and communications, facilitation, e-learning, distance education, community outreach and engagement, website development/maintenance, working with volunteers
- Fluent in French and English, an asset
