To support us on our journey, Sinai Health System is looking for a **Social Worker** to support our **Mount Sinai Academic Family Health Team (MSAFHT) Departme**nt at our Toronto Granvosky Gluskin Family Medicine Centre Site**. This position is specifically to support our mental health program for the perinatal population (including pregnant and postpartum people, families and young children).**
MSAFHT provides high quality evidence-based primary care to patients in an environment that supports the education of learners and the research and evaluation of primary care practices and outcomes. The Social Worker provides a range of social work services including assessment, crisis intervention, counseling and community referrals. The Social worker works collaboratively with the team and provides consultation to the team on managing issues that are psychosocial, emotional and mental health in nature related to our perinatal population.
To define and develop a culture and interprofessional primary health care practice model to enhance the delivery of patient centered care within an academic environment in the Mount Sinai community.
**Responsibilities**
- Working with the team clinical psychologist to develop a program addressing the needs of the perinatal population
- Assessment and social work diagnosis of psychosocial problems.
- Individual, couple, family and group counseling and psychotherapy.
- Case Management, including knowledge of and linkages to community resources.
- Health promotion.
- Psycho-education related to the prevention and management of mental health problems, including the facilitation of group sessions.
- Assistance in navigating service delivery networks and building relationships to find and secure required resources
- Advocacy to establish and access needed resources.
- Liaising with child protection agencies.
- Supporting patient self-management through the development of patient education resources/sessions.
- Actively contributing to the education of all learners in the academic setting.
- Implementation of standardized screening and assessment tools to support team delivery of mental health care.
- Development, management and delivery of programs alone or in collaboration with other professionals.
- Consultation with other professionals related to patient needs.
- Participation in case management rounds to refine treatment goals with the team.
- Participation in program development, planning, evaluation and clinical research in all aspects of the MSAFHT mental health program (e.g. assessment and care planning, crisis intervention and management).
- Perform other job related duties as required.
- Job Requirements- Successful completion of a Masters of Social Work from an accredited educational institution.
- Registration in good standing, with the Ontario College of Social Workers and Social Service Workers required.
- Minimum 3 years of recent direct client service experience in a community health care setting required.
- Demonstrated knowledge of current theoretical frameworks and social work practice standards such as CBT.
- Expert knowledge of community resources and proven referral practices.
- Demonstrated clinical skills working with a broad range of client groups and their families, particularly with the perinatal population and young children.
- Interest and experience in program and role development preferred.
- Able to function in a fast paced, changing environment.
- Previous experience with an electronic medical record (EMR), in particular PS Suite, is an asset.
- Demonstrated achievement of good working relationships with colleagues, professional staff, residents and the interprofessional team.
- Organizes time and set priorities effectively.
- Ability to work independently.
- Ability to make clinical decisions within professional domain.
- Demonstrated satisfactory work performance and attendance history.
