YMCA Northern Alberta is looking for a dedicated and compassionate Housing First Coordinator to join our Community team. This role offers comprehensive support to individuals and families as they transition from homelessness to long-term housing stability.

**Position Summary**:
The Housing First Coordinator provides client-centered, trauma-informed services to individuals experiencing chronic homelessness and high needs. The role spans from initial outreach and engagement to long-term follow-up support. Coordinators conduct housing assessments, help clients secure appropriate housing, and provide intensive case management after housing placement to promote stability and personal success.

**Key Responsibilities**:
**Outreach & Engagement**
- Conduct assertive outreach to locate and build rapport with individuals and families experiencing homelessness.
- Engage potential participants using a trauma-informed and strengths-based approach.
- Complete housing assessments and intake documentation for program eligibility.

**Housing Navigation & Placement**
- Coordinate with landlords and housing providers to secure safe and affordable units.
- Educate clients on tenant rights and responsibilities, rent budgeting, and apartment maintenance.

**Follow-Up Support & Case Management**
- Conduct regular home visits and wellness checks post-housing placement.
- Develop and update individualized service plans with each client to reflect changing needs and goals.
- Connect clients with supports such as mental health, addictions, employment, education, income support, and life skills development.

**Advocacy & Collaboration**
- Advocate on behalf of clients with landlords, service providers, and public systems.
- Collaborate with other YMCA team members and community partners to ensure coordinated care.
- Attend case conferences, team meetings, and community outreach events.

**Administration**
- Maintain accurate, timely documentation and case notes in accordance with organizational and funder requirements.
- Monitor client progress and outcomes to ensure program accountability.

**Qualifications**:

- Post-secondary education in Social Work, Human Services, or a related field (or equivalent experience).
- Minimum 2 years of experience working with vulnerable populations, particularly those experiencing homelessness, mental health and/or substance use challenges.
- Knowledge of Housing First principles, harm reduction, and trauma-informed care.
- Strong communication, relationship-building, and crisis intervention skills.
- Proficient in case management software and Microsoft Office Suite.
- Valid driver’s license and access to a reliable vehicle.
- Current First Aid/CPR and a clear Criminal Record Check with Vulnerable Sector Screening.
