**Social Service Coordinator - Full Time**

Glen Hill Strathaven is a 158 bed, long term care facility located in Bowmanville. We are currently recruiting for the full time position of Social Service Coordinator

To provide support to residents and their families including but not limited to, coordination of resident admissions in conjunction with the Director of Care, Business Coordinator, care team and Ontario Health at Home, orientation to the home, interpretation and follow-through of resident and family needs, referral of resident and /or family intervention as required.

The Social Services Coordinator is required to perform their duties in a manner consistent with the core values and employee guiding principles, ensure resident safety, and demonstrate customer service excellence.

**Highlight of Responsibilities Include**:

- Assists and supports new resident/substitute decision maker with admission into the Long-Term Care Home.
- Provide counselling where necessary and may refer to external resource for further counselling.
- Facilitates internal resident transfers in collaboration with the resident/substitute decision maker, home staff, and other residents.
- Establishes and maintains an effective quality and risk management system to ensure legislative regulations and standards are met;
- Ensures that measures are taken to facilitate optimal occupancy and occupancy numbers;
- Fosters positive working relationship with Ontario Health at Home and discharge planners;
- Provides discharge planning, support and co-ordination
- In collaboration with the RAI Coordinator, schedules and facilitates ongoing care conferencing;
- Models and fosters an environment of positive customer service in all aspects of care;
- Responds promptly to concerns of residents and family members, mediating a satisfactory response to concerns and implements action plans to _avoid _reoccurrence of similar issues;
- Facilitates communication in complex care issues to foster positive resident outcomes;
- Establishes and sustains care in a manner that is resident centered and fosters evidence based best practice;
- Assists in linking residents/family with resources and services both within the facility and community;
- Facilitates and maintains community counselling linkages and access for resident, families, staff and community;
- Facilitates family support groups, including liaison for the family council and family education sessions;
Qualifications:

- Must have at least two years relevant experience in social work or other related field.
- Must be registered with the Ontario College of Social Workers and Social Service Workers
- Must possess specialized knowledge in gerontology or any field related to seniors. Experience in a long-term care setting would be preferable.
- Must be mature and possess good communication and interpersonal skills. Must be in good physical and mental health.
- Must have experience in working as part of an interdisciplinary team;
- Must possess excellent leadership and communications skills;
