**KEY FUNCTION**:
Provide patient-centered therapy, manage equipment and supplies, and perform associated functions to support the delivery of service.

**REPORTING**:
Under the general direction of the Clinical Manager, Intensive Rehabilitation Unit, and under the supervision of the Speech Language Pathologist.

**DUTIES**:

- Provide treatment in fluency, articulation, aphasia, apraxia of speech, dysarthria, language impairments, cognitive communication, augmentative and alternative communication, and voice as prescribed by the Speech Language Pathologist.
- Work collaboratively with the Speech Language Pathologist to deliver an individualized treatment plan.
- Monitor and report the patient’s progress in treatment to the Speech Language Pathologist and other interdisciplinary team members.
- Report and document patient progress in meeting established objectives in treatment sessions.
- Prepare all necessary primary therapy materials and equipment for treatment.
- Maintain relevant statistical information.
- Reinforce strategies to help with communication disorders for the patient and family/caregiver.
- Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement in consultation with supervising Speech Language Pathologist.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Educate and promote health, safety and wellness in the work place.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a two (2) year Diploma as a Communicative Disorders Assistant, from an accredited college.
- Current Certificate of Registration with the Communicative Disorders Assistant Association of Canada (CDAAC).
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- Minimum of two (2) years’ experience working with communicatively impaired individuals within a health care environment.

**KNOWLEDGE/SKILLS/ABILITIES**:

- Demonstrated firm understanding of physiological and functional speech/language/hearing.
- Demonstrated knowledge and understanding of the Ontario Hospital Association/Ontario Medical Association Communicable Disease Surveillance Protocols.
- Demonstrated excellent judgment and proven analytical skills.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.

**PERSONAL SUITABILITY**:

- Successful Vulnerable Sector Check is required.
- Ability to use tact and discretion in dealing with health care providers and patients.
- Demonstrated ability to work effectively as a member of an interdisciplinary team.
