The Health Centre provides a broad range of services aimed at improving the health of individuals and the wider community. Our services include medical and dental care, health promotion, illness prevention and outreach programming. Our expertise and resources are focused on working with those who experience health inequities and barriers to accessing health services. Collaborating as an interdisciplinary team, we provide services and supports that address the social determinants of health including factors and barriers such as poverty, inadequate housing, food insecurity, substance use, language and culture, as well as stigma and social exclusion.
The Health Centre works with the most complex client population served by primary health care professionals within Waterloo Region. Our highly skilled and diverse team works from a strength-based approach with individual clients as well as the broader community. We collaborate with many partner agencies and organizations within Waterloo Region. Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers.

The Health Centre is currently recruiting for the following position:
**MANAGER - OUTREACH**
**Full-time Permanent - 1.0 FTE**
**Salary: $77,721.00 - $94,947.00 plus benefits & HOOPP Pension**
**Job Summary**:
The Manager of Outreach manages the day-to-day functions of Outreach. The Manager reports to and works closely with the Director of Health & Wellness with a focus on coordinating services to ensure efficient, effective and integrated client-centered approaches that meet our clients where they are at.
The Manager of Outreach also provides direct supervision for Outreach staff.
**Responsibilities**:
1. Coordinate all aspects of Outreach service delivery to ensure seamless and effective support for individuals and communities we serve.
2. Support the Director of Health & Wellness to manage and support the operational setup of the HART hub, ensuring its responsiveness to community needs.
3. Respond to day-to-day client, staff and operational issues.
4. Work with the Director of Health & Wellness to coordinate and support staff leads responsible for specific Outreach program.
5. Support the development and implementation of an annual operational plan for all Outreach locations.
6. Develop and implement effective communication channels including team meetings, digital tools and other communication methods. Ensure that clear communication pathways are in effect between Outreach staff and the Leadership Team.
7. Responsible for departmental reports.
9. Establish and maintain Community Health partnerships with external agencies and partners that help further the Health Centres Mission.
10. Oversees research requests and the Health Centre’s involvement in research activities while following organizational policies and procedures.
11. Participates in leadership team meetings.

**_Quality Assurance and Improvement_**
1. Ensure monthly reporting for all Outreach program areas.
2. Regularly review and analyze service delivery and other relevant data and provide interpretive information to staff and leadership to support decision-making.
3. Monitor workload and performance indicators, particularly those identified in accountability agreements with funders.
4. Provide leadership in the development and implementation of quality improvement initiatives related to Outreach programs and services.
5. Coordinate and implement chart audits and other quality assurance activities and provide leadership to ensure quality improvement.
**_ Human Resources_**
1. Support the Director of Health & Wellness in promoting a team-based approach within Outreach and across all Health Centre teams to enhance service delivery and community impact.
2. Support staff in understanding and implementing the quadruple aim to service delivery wherever possible.
3. Support, coach and supervise Outreach staff, including the development and monitoring of goals and individual work plans.
4. Approve staff requests for leave and professional development funding.
5. Complete performance appraisals and, performance improvement plans as needed, for all direct reports.
6. Participate in the recruitment and hiring of Outreach staff.
**_ Financial_**
1. Support the Director of Finance in developing and monitoring operating budgets and ensuring expenditures remain within budget.
2. Review and approve expenditures within assigned budgets.
**_ Other_**
1. Participate in the development, implementation, monitoring and evaluation of Outreach programs for individuals and client populations as determined by the Health Centre.
