**What You Will Do**:
The Case Manager is responsible for supporting and helping job seekers to clarify their needs and establish short term and long term goals. They develop service plans that identify the steps required to conduct an effective job search and meet their employment goal.
The Case Manager will ensure the positive promotion of the YMCA Employment Service; its programs and the image of the people we serve.
**Specific Responsibilities Include**:
- Functions with a high level of professionalism within the guidelines set forth for Employment Services
- Provide intake, assessment, and case management services for individuals experiencing barriers to employment
- Assess eligibility and suitability of participants for Employment Service programs and services, as determined by service guidelines, and provide appropriate services tailored to their needs
- Meet 1-1 with Clients to assess client needs that should include but not restricted to: Life Stabilization, career development, literacy and basic skills training, job retention requirements, specialized supports required, and disability accommodation
- Explore, identify and clarify client interests, abilities, skill, education and experience in relation to short and long term career and employment goals
- Create evidence based Employment Action Plans that incorporate labour market information to create informed decisions around career planning
- Provide appropriate information and support for provincial programs or supports such as Second Career and
Apprenticeship
- Preparation of job search tools such as cover letters and resumes and make appropriate recommendations encouraging a professional outcome
- Use motivational interviewing techniques to improve employment readiness
- Administer and interpret tests and assessments to determine interests and skills of clients
- Ensure wrap around support and the adaptation of services based on specialized need
- Make referrals and assist participants to access other services including, but not limited to, education and training opportunities
- Provides case coordination with specialized employment support agencies and/or community agencies and create referrals to community services
- Assist with the monitoring, follow up and support/guidance given to all clients throughout the pre-employment and post-employment cycle
- Complete, update and prioritize all required paperwork, case notes and documentation in a timely manner, including CaMS and ES Case documentation per YMCA privacy and confidentiality standards
- Meet or exceed an annually determined quota for service units while ensuring quality and holistic client service
- Assesses Client’s accessibility needs and identifies potential barriers; identifies appropriate client supports and makes connections with support organizations or vendors to implement resources for job success
- Develop additional resources necessary for a successful on the job experience in conjunction with community partners and colleagues - e.g. Grounding exercises, mindfulness, anti-anxiety tools, job pictures boards, tasks lists, and phone prompt reminders
- Initiate, coordinate and conduct marketing and outreach activities to community agencies, organizations, and employers
- Explore and develop avenues to help clients gain access to the labour market in traditional and non-traditional ways - job placements, job shadowing, mentorship, etc.
- Work collaboratively with colleagues to ensures employer is aware of clients’ needs and is supportive and cooperative
- Maintain updated knowledge of local, regional and broader labour market information
- Observe changing needs within the existing services and look for creative new opportunities to expand them
- Understand the Performance Management model and the Service Quality Standard and its composition, and actively work with the employment team to attain, maintain, and exceed that standard

**WORKING CONDITIONS**:
- Requirement of Standard First Aid and CPR
- Clear Vulnerable Sector check
**EDUCATION/TRAINING/QUALIFICATIONS**:
- Post-secondary education in social services, career counselling, education, or other related field.
- Trained in Motivational Interviewing techniques and Bridges out of Poverty philosophies
- Demonstration of proficiency with CaMS (Case Management Solution) or other system, which supports the delivery of client and employer information, is an asset
- Proficient in Outlook, MS Word, Power Point, Access, and Excel
