37.5 hours per week

Wage:

$27-$30 per hour, depending on experience

Reports to: Executive Director and CC Café Case manager/ Supervisor

The Housing Case Manager is a position that has been developed in partnership with Thrive Housing and Support with the intent to ensure that individuals with the lived experience of both non-traumatic and traumatic brain injuries are assisted to reach goals and activities that they hold to be meaningful and life-enhancing.

The Housing Case Manager is responsible for the support of BIAPR Clients who are living in a Thrive Housing unit to maintain successful housing for individuals who have otherwise been homeless or lacked appropriate housing. This includes responsibility for providing individualized support and intervention to individuals with ABI in the community through assessment, ongoing individual service planning and evaluation, and consultation with other service providers outside of BIAPR. Housing case management is a person-centred support system that assesses, plans, coordinates, and monitors services to help individuals maintain and thrive in stable housing by connecting clients to essential resources like healthcare, income support, mental health services, ultimately building independence and overall wellbeing.

The Housing Case Manager will:

· Lead individualized case management and service planning. Coordination with internal and external partners for BIAPR clients that are housed within Thrive Housing and Support.

· Deliver one-to-one and group-based support as needed.

· Provide training and information to external stakeholders regarding anti-oppressive, harm reduction, self-determination, and brain injury as required.

· Support a person-centered model of care with a strong focus on coordinated planning.

· Provide crisis intervention when appropriate.

Key ResponsibilitiesClient Support

· Build strong therapeutic rapport with the clients.

· Provide direct 1:1 support as needed.

· Collaborate with all BIAPR team members to develop and implement Service Plan Agreements and Lifestyle & Care Plans.

· Communicate with clients and family members as appropriate.

· Arrange and participate in cross-sectoral case conferencing.

Client Documentation

· Review client notes.

· Complete monthly and annual status reports.

· Track and upload timely documentation to internal databases.

· Monitor and update consent forms.

· Report and document all incidents in accordance with BIAPR's policies within 24 hours.

· Respect and protect the rights of clients, including but not limited to their right to independence, autonomy, and self-determination

Collaboration

· Attend Weekly staff meetings with Thrive Housing staff

· Provide relevant Brain Injury Training to Thrive staff as requested by Thrive supervisors.

· Serve as backup staff support when necessary.

· Cross-sectoral case management with other service providers including but not limited to social workers, medical staff, corrections, law enforcement, social services.

· Internally coordinate with Community Service Coordinators and Recreation Therapists to create and update goals with clients.

· Assist clients in navigating complex medical, financial and social systems through advocacy, collaboration, motivational interviewing, and skill development.

Qualifications

· Post-secondary education in Social Work, Social Services, Mental Health/Addictions, Housing and Homelessness, or related field.

· Minimum 3 years' experience providing community-based support, ideally to individuals with brain injury, mental health concerns, addictions, or housing instability.

· Strong knowledge of psycho-social and rehabilitative practices.

· Solid understanding of Privacy Information and Protection Act (PHIPA).

· Understanding of local community services and resources.

· Ability to maintain professional boundaries using a strength-based, client-centered approach.

· Commitment to client rights, autonomy, and diversity, equity, and inclusion.

· Excellent communication, time management, and interpersonal skills.

· Proficiency in Microsoft Office and willingness to learn new systems.

· Valid Ontario Driver's License, reliable vehicle, and insurance as per agency policy.

· Satisfactory Police Record Check and Vulnerable Sector Screening.
