**Role Overview**

We are seeking Mental Health Case Manager in a Focus Team who will be completing a comprehensive mental health and physical health assessment of the nature of each client’s unique goals and needs through collaborative engagement with each client and their respective support networks.

**About the Team**

The Focus is an innovative partnership program between St. Michael’s Hospital Mental Health Service and Cota. The Focus will rely on an interdisciplinary team approach to provide a continuum of integrated intensive case management (ICM) and Assertive Community Treatment services to meet the needs of clients with mental health and complex care needs in the Mid-East Toronto Health Link. Services to clients of the ISCT will be organized to allow for flexibility of movement between levels/intensity of services (Intensive Case Management and ACT) while ensuring continuity of care for clients.

**Primary Responsibilities**:

- Contribute to the development of comprehensive client-centered assessments and treatment plans.
- Develop collaborative, supportive and therapeutic relationships with clients.
- Completing a comprehensive mental health and physical health assessment of the nature of each client’s unique goals and needs through collaborative engagement with each client and their respective support networks.
- Provide case management functions: support, guidance, linking and assistance to clients in relation to the development of personalized Client Safety Plans, Recovery/Goal Plans, Ontario Common Assessment of Need (OCAN) and other assessments as required.
- Provide health education to clients and team to promote health and well being.
- Provide direct practical support, coaching and counsel to clients in their pursuit of identified goals and needs.
- Provide education, information and support to promote hope and the pursuit of personal recovery.
- Work collaboratively as a member of the inter-professional team.
- Assist clients with emergency and/or crisis needs as required/appropriate.
- The Case Manager, Mental Health will also assume responsibilities for coordinating and/or facilitating client group sessions.
- A combination of a BSW, MSW or other regulated health care professional designation and a minimum of one (1) year addictions experience, OR
- A combination of a post-secondary degree/diploma with four (4) years addictions experience in the delivery of mental health services.
- Preference will be given to individuals who have pursued specialized education/training/certification in the field of mental health, for example: principles of recovery oriented practice, certificate in Mental Health Case Management, ASIST, group facilitation, trauma informed counselling, mindfulness based counselling.

**Other Requirements and Working Conditions**:

- Conduct visits in the clients’ homes, hostels, hospitals and other community. settings working in sometimes difficult conditions, including hoarding.
- Be comfortable working with a broad range of client groups.
- Work rotating day, evening and weekend shifts.
- This position is 8:00am - 4:00pm with weekends and on call overnight.
