**Requirements**:

- Graduate of Recreation Facilitator of Older Adults Certificate or equivalent.
- Non-Violent Crisis Intervention training.
- At least one year of recent experience working with individuals/groups in a healthcare environment
- Demonstrates knowledge and support of:

- The vision, values, goals, and objectives of the Southeast Personal Care Home.
- Personal Health Information Act, Protection of Persons in Care Act, Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices (Universal Precautions) and other legislated acts.
- Knowledge of current trends and practices in recreation and leisure services for frail elderly.
- Knowledge of community recreation facilities and services.

**Responsibilities**:

- Assesses the leisure needs of patients/residents on assigned units and documents results for use by the interdisciplinary team.
- Establishes and reviews treatment goals and objectives collaboratively with the patient/resident.
- Plans, organizes, implements and evaluates therapeutic recreation/activity programs for individual patients/residents in accordance with their age and social, physical, mental, spiritual and leisure needs.
- Responds to requests for service; based on an assessment and goals established with the patient/resident, recommends participation in the appropriate therapeutic recreation services.
- Develops and maintains effective oral and written lines of communication with Managers, Recreation Coordinators, colleagues/co-workers, and facility and community resource personnel.
- Communicates on patient/resident assessment and outcomes at interdisciplinary team meetings and in health records in accordance with protocols.
- Participates in the evaluation of the effectiveness of therapeutic recreation/interventions and services offered to patients/residents.
- Maintains daily attendance records for patients/residents during programs.
- Maintains assigned recreational space.
- Responsible for the safety of patients/residents and volunteers at all times, ensuring that adequate assistance is available for patient/resident groups and outings.
- Promotes quality patient/resident and family interactions and leisure education opportunities.
- Attends scheduled in-service programs and professional development opportunities (i.e. seminars, lectures, films, workshops, etc.) as requested.
