Mental Health and Dementia Coordinator for St. Joseph’s Health Centre
Permanent Full Time $39.58-$52.86 per hour

**Mental Health and Dementia Coordinator** will lead and oversee the development and sustainability of our mental, emotional, and spiritual wellness programs within our long-term care homes. This role plays a key part in delivering person-centred care to meet the needs of our residents. The coordinator will inspire and promote wellness programs, collaborating with the healthcare team to ensure high-quality care and optimal outcomes for residents.
**Responsibilities Include**:
- Lead and coordinate mental health and dementia programs across SJHC long-term care homes, through research, planning, development, and evaluation.
- Provide administrative and operational support to the Director(s) of Care regarding mental health and dementia programming.
- Participate in strategic planning, creating short
- and long-term goals for mental health and dementia programs.
- Develop and implement systems and processes to ensure the sustainability of mental health and dementia initiatives.
- Establish and monitor best practices and quality standards for mental health and dementia care.
- Develop, implement, and revise policies and procedures to improve care standards.
- Facilitate coordinated care across various service continuums within SJHC’s long-term care homes.
- Build and maintain collaborative relationships with community partners, staff, students, and volunteers.
- Travel between sites to monitor and support the implementation of the mental health and dementia programs.
- Assist with performance management activities, including attendance support, performance audits, coaching, and progressive discipline.
**Qualifications**:
- Bachelor’s degree in a health-related field.
- 1-2 years of experience working with individuals and families in the mental health and dementia systems, with preference for experience in Long-Term Care.
- Knowledge of Mental Health legislation, Occupational Health & Safety Act, Human Rights Code, Fixing Long Term Care Act, Privacy Legislation, and Mental Health Policy considered an asset.
- Strong critical thinking, analytical decision-making, and problem-solving skills.
- Proven expertise in professional practice, quality standards, and the ability to teach, mentor, and coach teams.
- Excellent interpersonal, communication, planning, and time management skills.
- Education or certification in Trauma-Informed Care, Mental Health First Aid, SafeTalk, Applied Suicide Intervention Skills Training, Nonviolent Crisis Intervention, Gentle Persuasive Approaches, or similar programs is an asset.
- COVID Vaccine Mandatory to work in our facilities (2 Doses)
