Job Title: Manager for the Interprofessional Primary Care
Location: Oakville, ON
**Job Type**: Full Time, Permanent
Pay Rate: $65-70k/annum.
**Job Overview**:
We at Agile Healthcare Solutions are seeking a skilled and compassionate Manager for the Interprofessional Primary Care Team to join our healthcare team. In this position, you will play a crucial role in leading an IPCT Team of Nurses, Nurse Practitioners, Social Workers, and Support Workers in assisting communities in operations around the Halton Region.
**Responsibilities**:
- Lead the IPCT in delivering one-to-one primary care and clinical support, meeting clients where they are, including shelters, supportive housing, and street-level outreach.
- Provide harm reduction support, comprehensive medication support, and coordination with other healthcare partners.
- Navigate solutions to barriers to participation in social roles and meaningful activities.
- Support discharges and intakes from/to hospital mental health units, including Form 2 processes.
**Qualifications**:
- Regulated Health Care Professional (MSW or RN preferred).
- 5+ years of leadership/management experience, preferably in non-profit or healthcare sectors.
- 10+ years of direct clinical experience in mental health and/or addictions.
- Strong ability to build genuine connections and advocate for clients.
- Experience in program development, quality improvement, and strategic planning.
- Trauma-informed, with expertise in harm reduction, crisis intervention, and risk assessment.
- Excellent communication and documentation skills.
- Valid driver's license and vehicle access.
- First Aid & CPR certification; Naloxone and ASIST training preferred.

Pay: $65,000.00-$70,000.00 per year
Schedule:
- 8 hour shift
- Day shift
- Monday to Friday
- Morning shift
**Education**:
- Master's Degree (preferred)
**Experience**:
- Nursing: 1 year (preferred)
- MSW: 1 year (preferred)
- Harm Reduction Supports: 1 year (preferred)
- Leading IPCT Team: 1 year (preferred)
Licence/Certification:
- Driving Licence (preferred)
