**What do you need to be successful?**
- You are a motivated self-starter with superior technical, verbal, and written communication skills
- You enjoy providing community based clinical services as part of a multidisciplinary trauma rehabilitation team, in clients' homes
**Qualifications**:
- You have a Master’s degree in Social Work with a minimum of two years clinical experience
- You are currently registered with the Ontario Association of Social Workers
- You have a valid Drivers Licence and access to a vehicle
**What is a day in the life of a FA Social Worker/Case Manager?**
- You will provide assessment and counselling to clients who have experienced trauma, traumatic brain injuries, spinal cord, and complex orthopedic injuries
- You will interface with the medical, legal and insurance systems
- You will connect clients with a variety of relevant community-based resources and services
- You will advocate for and help clients access community-based resources that would improve their well-being
- You will monitor, evaluate, and record client progress according to rehabilitation goals described in treatment and care plans
- You will provide detailed, comprehensive reports as part of the medical-legal system
- You will maintain professional and technical knowledge by attending training sessions and establishing personal networks
