Your Opportunity This Social Worker II position based at the Cold Lake Community Health Services provides social work services as part of an inter‑disciplinary team to clients in acute care, continuing care and the community homecare settings at the Cold Lake Community Health Services, Cold Lake Healthcare Centre and Bonnyville Community Health Services. Key components of this position are social work support for AHS system navigation, placement/transitions in care, and financial supports. Description As a Social Worker II, you will be a member of an inter‑disciplinary team and provide a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource‑based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications Certification in Basic Life Support – Healthcare Provider from the Canadian Heart and Stroke Foundation. Preferred Qualifications One year experience with acute care, continuing care and/or homecare experience. Enhanced knowledge of community‑based supports and services.
