**INTRODUCTION**:
The MH Adult Outreach Worker will provide case management supports for adults ages 18+ experiencing mental health and addictions issues to access other health and wellness services in both Hamilton and Brantford regions. This role will work with both western and Indigenous services providers to help adults, caregivers, and their families to better navigate mental health and addictions services and manage mental health and addictions issues; all working towards improving patient outcomes.

**Required Knowledge, Skills, and Ability**
- Knowledge and ability to integrate wholistic, strength and evidence-based practice into the planning and evaluation of client and program intervention from a trauma informed lens
- Experience in mental health and substance use treatment planning; therapeutic interventions, life skills training, coaching, advocacy and support to clients and their caregivers/family
- Knowledge of community resources, specifically Indigenous programs and services in Hamilton and surrounding area;
- Knowledge of Indigenous cultures, customs, and traditions; history of colonization and intergenerational traumas impacting the health and wellness of Indigenous Peoples.
- Knowledge of mental health and addictions counselling models, techniques, modalities, and approaches.
- Knowledge and understanding of pathways of care for various mental health and addictions issues.
- Ability to work independently, as well as part of a team.
- Possess excellent time management, organization, and communication skills.

**STATEMENT OF QUALIFICATIONS**:

- Post secondary degree or diploma in social work, psychology, &/or concurrent disorders or related field
- 1+ years experience working with individuals with complex needs
- Experience working with and extensive knowledge of Indigenous Peoples language, culture, customs and traditions.
- Must be able to work in both Hamilton and Brantford offices.

**Job Types**: Full-time, Fixed term contract
Contract length: 3 months

Pay: $48,000.00-$50,000.00 per year
