Outreach Worker

St. Matthew’s House (SMH) is a charitable not-for-profit multi-service agency. Services include emergency food, crisis intervention and outreach to seniors and vulnerable persons to support independent living. Clients include persons with limited or no income, persons living in poor health or with chronic conditions, persons of all ages and newcomers.

Outreach provides temporary services to people who are experiencing homelessness and typically disconnected from the system. The Housing Focused Street Outreach Team is an important part of the emergency response in the homeless-serving system to support connection to housing through coordinated access.

Reporting to the Manager, Seniors Programs, St. Matthew’s House and the Supervisor, Housing Focused Street Outreach Team (City of Hamilton, Housing Services Division) the Housing Focused Street Outreach Worker will connect homeless individuals and households who are sleeping rough or in places not fit for human habitation to obtain shelter, services, and housing. Street Outreach staff will assist clients by providing advocacy and case management for the purpose of obtaining housing. Street Outreach staff will also support with encampment responses.

**Qualifications**:

- Experience working with individuals living in homelessness.
- Primary case management and assessment skills regarding a variety of needs usually acquired through post-secondary education or experience in psychiatric/mental health services and other community programs.
- Knowledge of community resources and services, knowledge of homeless-serving system and housing resources.
- First Aid/ CPR certification; Non-violence Crisis Intervention Training is an asset
- Valid drivers license and access to a reliable vehicle is an asset

**Job Types**: Full-time, Part-time

**Salary**: From $24.49 per hour
