Mount Sinai Hospital Wellness Centre -A Community Mental Health Program for Seniors and Caregivers.

**Summary**

**Responsibilities**

  • Performing initial screening of clients, obtaining client consent, conducting preliminary assessment interviews, and reviewing clients' psychosocial situation and history.
  • Providing assessment, treatment, continuous illness management, medication monitoring as well as caregiver support and training, either at the clinic or at patients/clients’ home, to enhance the feasibility and length that the patients/clients can be cared for at their home in the community.
  • Identifying and counseling individuals at risk for developing serious mental illness.
  • Participating in treatment planning, its implementation and outcome evaluation.
  • Consulting psychiatrist and program manager in assessment, treatment and case management when necessary.
  • Communicating with physicians, referring agencies, families, community organizations with regards to clients' situations or case planning.
  • Offers individual counseling, group and family therapy as part of the clinical intervention.
  • Providing clinical outreach to clients and families who face challenges or difficulties to get out of home to receive needed psychogeriatric care or services and those who are socially isolated.
  • Working in conjunction with families, caregivers and community partners, to early identify clients/patients in the community who are having a psychogeriatric concern, so that timely assessment and intervention become possible.
  • Providing support/training to families in handling/resolving issues related to patients' needs.
  • Referring clients to MSH joint programs, community partners, community agencies, and self help groups and supporting them in their treatment.
  • Maintain proper documentation using an electronic health record to document including intake assessment reports, progress notes and treatment plans.
  • Offering clinical consultation and education to front-line staff of social and seniors service agencies who require specialized support in addressing and handling psychogeriatric issues.
  • Exploring and developing new program initiatives with community partners and collaborators, including clinical and cultural consultation, to increase the overall capacity to reach out to and support clients/patients and families who are facing a psychogeriatric issue.
  • Liaising with community partners and outside agencies to explore service collaboration and identify service needs.
  • Organizing and coordinating health/mental health education and wellness programs.
  • Participating in research and academic activities which benefit the service delivery and program development. Supervises placement students related to own profession.

**Please note that this role involves some in-city traveling and off-site visits.**

  • Job Requirements- Completion of a Master’s Degree in Social Work or Occupational Therapy or Bachelor of Science degree in Nursing.
  • Current member in good standing with the appropriate governing college.
  • Minimum of 3 years of clinical experience in the care of seniors with mental illness.
  • Knowledge of community mental health resources required.
  • Clinical experience in general psychiatry, crisis and psychosocial intervention is required.
  • Ability to provide field education to practicum students of a related discipline, when assigned.
  • Must be a self-starter, work well independently and within a dynamic team, and should be able to demonstrate skilled ability to work with a variety of patients across the spectrum of mental illness.
  • Excellent organizational, communication (verbal and written), computer literacy, and interpersonal skills.
  • Experience with suicide risk assessment required.
  • Demonstrated satisfactory work performance and attendance record.
  • Must be fluent in Cantonese and Mandarin and have the ability to deliver culturally competent care to effectively address the psycho-geriatric needs of the patients visiting the clinic.
