Position Overview
**SUMMARY**:
Reporting to the Director of Clinical Operations, the Patient Flow Manager (PFM) assumes a leadership role in promoting quality of care, patient experience and change. As part of the role, this position works closely with all team members of discharge planning team, various other health professionals, and support staff, to facilitate and improve patient flow at HGH. This position is responsible for coordinating and facilitating activities related to the admission, transfer and discharge of patients as well as navigation outside the hospital. The incumbent liaises between all clinical services and develops proactive strategies to address patient flow, using an equitable and collaborative approach and ensuring respect and commitment from all parties involved. The primary goal of the Patient Flow Manager is to facilitate and enhance patient flow organizationally and to mobilize teams to achieve throughput. The PFM does this with the goal of providing an exceptional experience for patients, families and staff. The PFM understands the demand and capacity challenges of a hospital system and works to achieve patients being in the right place, at the right time, receiving the right care.
**Education**:
Bachelor's degree in Nursing preferred or a bachelor’s degree in a related field such as health administration or social services.
**Experience**:
A minimum of 3 years of experience in middle management.
Recent clinical experience in patient flow/ bed management
Member of the College of Nurses of Ontario or current registration with relevant professional college.
**Skills**:
Demonstrated agility in decision making; ability to think critically in order to respond to a rapidly changing environment.
Ability to assess risk, and effectively utilize staff resources throughout all sites to support patient acuity and skill level of staff members while considering the global needs of all patients
Ability to listen, engage, collaborate, negotiate, facilitate and entertain creative solutions in order to enhance patient flow organizationally and mobilize teams to achieve throughput
Ability to foster collaboration in an interdisciplinary team environment
Effective time management skills.
Excellent interpersonal relationship and communication skills (oral and written).
Strong negotiation and conflict resolution skills.
Demonstrated ability to balance multiple and competing priorities in a fast-paced environment.
Highly computer literate and skilled in use of MS Office.
Criminal Check
**As a pre-condition of employment**:
All new HGH employees will be required to obtain and provide a satisfactory Police Records Check dated within six (6) months from the date of issue.
As of now, HGH will require all employees, physicians, learners, contractors and volunteers to be fully vaccinated against COVID-19.
