**_GENERAL ACCOUNTABILITY: _**
Reporting to the Manager, Population Health, responsible for the Lung Screening and Prevention (LSP) Program, the Smoking Cessation Case Coordinator is responsible for assessing program participants’ readiness for smoking cessation, identifying their needs, and providing client-centred support, counseling, and referrals to appropriate services. The position also conducts follow-up assessments, tracks progress, and celebrates client achievements to encourage long-term cessation success.
This role actively contributes to the development, implementation, and evaluation of program strategies and projects aimed at improving service delivery within the Lung Screening and Prevention Program. This includes supporting training and education initiatives to enhance service coordination, quality, and effectiveness. The position works collaboratively with internal and external stakeholders to ensure the program's objectives are met.

**_Education: _**
- University Degree in a health or human services field.
**_Experience: _**
- Two (2) years experience counselling or providing interventions to clients in a mental health or addictions setting.
- Experience in managing a client or patient case load, including assessing needs, making plans and following up on progress.
- Experience with smoking cessation interventions or programs is an asset.
- Experience with client record and management software is an asset.
**_Knowledge and Skills: _**
- Demonstrated ability to work collaboratively with multiple, diverse stakeholders at all levels of responsibility both within the SCA and externally.
- Experience in providing evidence-based, person-centered, trauma informed, and culturally safe supports for clients dealing with addictions or other mental health concerns.
- Goal oriented and customer service focused, with a solution-based approach to managing client needs.
- Strong motivational interviewing and counseling skills.
- Ability to work in a discreet and confidential manner while being adaptable to a changing environment.
- Ability to assess, create, and adapt ongoing client support plans.
- Proficiency in case management (software) and associated documentation.
- Awareness and understanding of how various factors such as gender, sexual orientation, geography location, socio-economic status and mental health can influence a person’s experience with nicotine addiction and tobacco use.
- Ability to form trusting therapeutic relationships with clients.
- Excellent verbal and written communications skills, with the ability to capture information during and after client interactions in a software system.
- Strong problem solving and use critical thinking skills to provide timely and appropriate support clients.
- Works well independently and is highly organized and manages time effectively, with ability to manage multiple tasks in a timely manner.
