GENERAL ACCOUNTABILITY:
Lung Screening and Prevention Program (LSP) Navigator is responsible for the coordination, development, implementation, administration, evaluation, and delivery of the client navigation component of the LSP program. In collaboration with provincial health system partners, clinicians, and community partners, the Navigator acts as a clinical and health promotion resource, providing support, leadership, direction and education to promote the coordination and continuity of services in matters related to the LSP Program. This position reports to the Manager, Population Health.
QUALIFICATIONS:
Education:
- Bachelor Degree in Nursing
- Oncology nursing certification an asset
Experience:
- Five (5) years' nursing experience.
- Demonstrated experience in leadership, program development and evaluation, and patient education required.
- Respiratory and/or Oncology nursing experience working with patients' with chronic respiratory illnesses or lung cancer at various stages of their treatment journey.
- Experience with, or knowledge of, the delivery of smoking cessation interventions or programs.
Membership:
- Licensed with the College of Registered Nurses of Saskatchewan (CRNS).
Knowledge and Skills:
- Demonstrated ability to work collaboratively with multiple diverse stakeholders at all levels of responsibility both within the SCA and externally.
- Demonstrated ability to provide person-centered, trauma informed, culturally safe nursing support.
- Advanced client/patient assessment skills with the ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning) and provide high quality client support through appropriate/prescribed technical, therapeutic and safety interventions.
- Ability to interpret detailed information and communicate effectively both orally and in writing.
- Requires high degree of judgment and problem solving ability and critical thinking skills to identify, analyze and define problems and determine the most appropriate solution.
- Ability to integrate relevant evidence-based best practice as standard performance within the program.
- Ability to teach clients/patients over the telephone and in person.
- Ability to teach and present information effectively to persons over whom the incumbent may have no direct authority.
- Requires high-level skills in human relations to develop formal and trusted relationships with clients to navigate them effectively within the provincial health system and provide in-depth counselling to support clients in smoking cessation.
- Promotes primary prevention and early detection through health professionals, community-based programs and local support group
- Demonstrated leadership skills required to lead Agency projects/committees.
- Ability to manage and troubleshoot multiple tasks in various stages of completion according to priorities.
- Competent in the use of computers and Microsoft applications.
- Maintains confidentiality.
- Knowledge of culturally responsive care for First Nations and Métis people.
