**Right at Home Halton/Niagara **is looking for a **Full-time** **Care Coordinator (Scheduler) **to add to our **Niagara** team.

This is a Hybrid position, working from home with periodic in-person team meetings in Hamilton.

**Job Summary**:
**Qualifications**:

- Completion of a recognized college program in office administration/medical office administration or equivalent office experience.
- 1 year care coordinator scheduling experience in a busy healthcare environment preferred
- A minimum of 2 -3 years of related experience, preferably in the healthcare field.
- Ability to multitask while working in a fast-paced environment with changing priorities
- Ability to work both independently and as part of a team
- Ability to organize and prioritize work effectively
- Ability to express ideas clearly in both written and oral communications.
- Excellent interpersonal skills
- Proficient with Microsoft Office & computer skills
- Experience with AlyaCare HealthCare Scheduling softward an asset

**Responsibilities**:

- Actively works on filling urgent client requests and caregiver calls.
- Contact family members and/or appropriate Care Planners for all urgent and emergency situations. Follow process and guidelines
- Attend weekly phone meetings

**Salary**: $20.00-$22.00 per hour
