We are currently seeking a **Care Coordinator** to support our growing team in delivering excellent service and seamless care coordination.

**Role Overview**:
The Care Coordinator serves as the primary liaison between clients, caregivers, families, and healthcare professionals. This role ensures that care plans are effectively implemented, schedules are maintained, and service quality meets both client expectations and regulatory standards.

**Key Responsibilities**:
**Care Coordination & Client Support**:

- Conduct intake assessments for new clients
- Develop, review, and update individualized care plans in consultation with families and staff
- Monitor client satisfaction and service quality
- Ensure smooth onboarding and matching of caregivers to clients based on care needs and preferences

**Scheduling & Staffing**:

- Manage caregiver schedules and ensure coverage for all shifts
- Handle last-minute changes and replacements when needed
- Maintain accurate records of assignments and timesheets

**Communication & Liaison**:

- Serve as a point of contact for clients, families, and caregivers
- Respond promptly to inquiries, concerns, or issues
- Coordinate with healthcare providers and community partners when necessary

**Compliance & Documentation**:

- Ensure all documentation is complete, accurate, and in line with privacy and health standards
- Support adherence to internal policies, safety protocols, and licensing requirements
- Maintain updated client and caregiver files

**Administrative Support**:

- Assist with reporting, caregiver onboarding, and training support
- Coordinate wellness initiatives or community engagement presentations when needed

**Qualifications**:

- Experience in healthcare, social work, or home care coordination (HCA, LPN, or related background preferred)
- Excellent communication and organizational skills
- Compassionate, reliable, and professional demeanor
- Ability to work independently and manage multiple tasks
- Proficient with scheduling systems, Microsoft Office, and basic digital tools
- Bilingual is an asset
