Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services. We provide residential, day treatment, and community-based services including in-home and school-based programs to children, youth, and families within the community.

Lynwood Charlton Centre is the Lead Agency for the Hamilton service area for Moving on Mental Health. This is a Ministry initiated plan to transform the experiences of children, youth and families dealing with mental health issues, and strengthen the community-based system for delivering mental health services to them. Through our Lead Agency role, we are responsible for leading and facilitating system reform in Hamilton. To do this, we work collaboratively with other child and youth mental health service providers, other affiliated community partners including our child welfare, health, and school board partners, and parents and youth engaged in our services.

**Role Overview**

Directly reporting to the Senior Director, Systems Planning, the Manager, ASN works collaboratively to implement and manage a single point of access and a system of navigation for children and youth mental health services in the Hamilton area.

**Main Duties and Responsibilities**
- Lead the development and implementation of the plan for access and system navigation to children and youth mental health services in alignment with community plans, LCC’s strategic plan, accreditation standards, and legislative requirements.
- Collaborate with Agency and community partners to identify and address potential barriers to success of the program, and to ensure effective messaging to all impacted partners.
- Track program development and effectiveness and provide monthly updates to the steering committee; participating fully in all quality improvement and program evaluation initiatives.
- Ensure timely execution and compliance with MOHLTC and MCCSS reporting requirements.
- Resolve related escalated concerns and complaints from families.
- Support oversite of the client information portal (opening and closing clients, reporting of client statistics, etc.)
- Contribute to the development of the Access and System Navigation team.
- Provide leadership and oversight to the Access and System Navigation team.
- Provide Agency wide access and system navigation training initiatives.
- Ensure all related health and safety responsibilities are addressed.
- Participate in the rotating on call manager schedule for the Agency.
- Other related duties as assigned.

**Experience/Skills and other Requirements**
- Minimum of a Bachelor of Social Work degree (B.S.W.) or equivalent required; Masters’ degree preferred.
- Current registration (in good standing) with one of the following professional colleges preferred: Social Workers, Occupational Therapists, Nursing, or Psychology.
- Minimum of 3 years’ experience at a supervisory or management level within the children’s mental health, or similar setting required.
- Minimum of 3 years of experience in case management, care navigation, or health planning preferred.
- Minimum 5 years of direct experience in a children, youth, or adult mental health setting preferred.
- Strong understanding of relevant best practices in clinical assessment, client service approaches, and program design.
- Proven ability in agency-wide/system-wide change initiatives.
- Proven Quality Improvement (QI) planning and program implementation experience.
- Strong technological acuity required to ensure systems operate effectively and in concert with one another is an asset.
- Demonstrated ability to build strong relationships with key stakeholders, including Ministry representatives, local and provincial community partners, youth and families, and senior leaders across multiple organizations, both virtually and in-person.
- A valid G class drivers license, and reliable vehicle is required, as this role requires local work-related travel within Hamilton. Minimum 1 million dollar liability insurance coverage is required.
- Satisfactory vulnerable sector background check required.
