Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services, including congregate care, day treatment, community-based services including in-home and school-based services, to children, youth, families and the community.
The Centre is funded and licensed by the Ministry for Children and Youth Services and accredited by the Canadian Centre for Accreditation. We are currently looking for **Full Time Mental Health Clinician, Social Worker** for our Central - Upper Paradise Site to join our team.
**Role Overview**
Working under the direction of the Manager of GPS/Thrive, Clinical Director, the Mental Health Clinician, Social Worker is responsible to assume direct clinical responsibility for cases as assigned, including fulfilling assessment, case conceptualization, case presentation and management, and providing direct service to children, youth, and families. The Mental Health Clinician/Social Worker will also perform duties related to program development and evaluation, through developing effective linkages with service providers from larger systems in the community and act as a resource and clinical lead to clinical interdisciplinary teams as needed.

**Main Duties and Responsibilities**
- Provide direct service/counselling to the children, youth and families in their home, school and/or community locations.
- Utilization of biopsychosocial assessments to formulate treatment plans.
- Participating in the intake/screening and ongoing case management for children/youth referred to clinical programs.
- Participate in clinical team, including intake, implementation, program development and evaluation.
- Provide resources, and treatments utilizing a wide range of evidence-based treatment modalities appropriate to the needs and strengths of children, youth and families.
- Maintain clinical records, assessments and reports according to agency, program policy, procedures, and regulatory requirements.
- Document assessment, plan, interactions, and interventions according to agency guidelines and standards.
- Maintain records and statistics in accordance with agency policies. Attend and participate in staff, committee, department, and other administrative meetings.
- Facilitation of group work for children, youth and families.
- Providing clinical consultation to community partners, schools or other programs that involve the clients.
- Liaise and support referrals with community agencies on behalf of the children, youth and families.
- Provide clinical leadership in the treatment planning process and evaluation of treatment outcomes.
- Attend and participate in team meetings.
- Contribute to the development, implementation, and evaluation of clinical programs.
- Contribute to effective team development and processes to promote team cohesion and communication.
- Participation in all mandatory agency training and in-service processes.
- Assume responsibility for own professional development, including keeping abreast of new techniques in the field, community resources and acquiring therapeutic modalities required to best serve the children, youth, and families within the Program.
- Facilitate training sessions for staff in collaboration with the clinical director and program managers.

**Experience Required**
- Several years of progressive experience working with families and children and/or youth in a treatment/counselling setting.
- Experience working with and developing intervention plans aimed to enhance client functioning within a school setting.
- Experience working with children/youth in a school setting.
- Experience collaborating with an interdisciplinary team to set and achieve common goals.
- Experience with navigating and/or collaborating with other community systems.
